Study On The Fertility Desire And Influencing Factors Of HIV-infected People In Cross-border Marriages In The China-Burma Border Of Yunnan Province

Abstract/Summary:
ObjectivesTo understand the fertility desire of HIV-infected people in cross-border marriages in the China-Burma border of Yunnan province,and to analyze the influencing factors of fertility desire.MethodsThis study is a cross-sectional study using a combination of quantitative and qualitative research.From April to September 2017,adopting purposive sampling and taking county as a unit,a cross-sectional survey was carried out in all the HIV infected people who were reported,could be followed up and effective communication in Longling county,Tengchong county,Mangshi City,and Yingjiang County in Yunnan province.Using self-designed questionnaires to conduct quantitative studies by asking HIV-infected people on fertility intention information.Using absolute numbers and percentages to statistically describe the basic information distribution,use a chi-square test and non-conditional logistic regression model to analyze the influencing factors of fertility desire.Qualitative research is conducted through personal in-depth interviews with HIV-infected individuals and medical workers.Use Epidata3.1 to input data and SPSS 22.0 for statistical analysis.Results1.241 HIV-infected people were investigated.The average age of HIV-infected people is 36.75 years old,and 23.7%indicated that there would be a future fertility plan.In the ideal number of children 71.4%(172/241)indicated that they wanted to have 2 children,12.9%(31/241)wanted to have 3 children,12%(29/241)wanted to have 1 child.17.8%(43/241)of HIV-infected people had a gender preference for children.2.1n the aspect of core knowledge of mother-to-child transmission and prevention,the awareness rate of HIV-infected people were 75.9%(183/241),75.9%(183/241),79.3%(191/241),and 85.1%(205/241)respectively on "whether the HIV will be transmitted to the fetus through maternity when pregnant","whether or not childbirth will cause the child to become infected","whether drugs can control mother-to-child transmission","whether or not breast-feeding can cause children to become infected".3.The influencing factors of fertility desire of HIV-infected people included age,family annual income,number of existing children,whether there is a gender preference for children,and MTCT knowledge awareness on "whether the HIV will be transmitted to the fetus through maternity when pregnant".4.Older HIV infected people who want to have children in the future are 0.16 times younger(OR=0.16);the probability of HIV infected person who has a family income of more than 10 thousand RMB to have a child in the future is 2.55 times as much as those who has a family income of less than 10 thousand RMB(OR=2.55);the probability of HIV infected person with only 1 child in the future is 0.05 times less than those who have no children(OR=0.05),and the probability of having 2 or more children is 0.01 times less likely to have children in the future than those who have no children(OR=0.01),and a HIV infected person with sex preference for a child who wants to have a child in the future is 4.43 times more likely than those who have no sex preference for a child.(OR=4.43);HIV infected people who know MTCT knowledge,whether HIV will be transmitted by mother to the mother,is 13.14 times more likely to want to have children in the future(OR=13.14).5.Qualitative interview results are consistent with quantitative analysis results.Other reasons included the concept of traditional succession,the knowledge of HIV mother-to-child blocking techniques,the fear of exposing HIV status,and the desire to have a biological child with the current spouse.Conclusions1.23.7%HIV-infected people in cross-border marriages in the China-Burma border of Yunnan province indicated that there would be a future fertility plan.The level is relatively lower.In the ideal number of children 71.4%HIV-infected people indicated that they wanted to have 2 children,17.8%had a gender preference for children.2.The influencing factors of fertility desire of HIV-infected people included age,family annual income,number of existing children,whether there is a gender preference for children,and MTCT knowledge awareness.3.It is suggested that full consideration should be given to the fertility desire of HIV-infected people in cross-border marriages in the China-Burma border of Yunnan province in future work.Strengthen the follow-up management of HIV infected people at childbearing age with a willingness to give birth.At the same time,effective form of PMTCT health education should be carried out.Provide fertility consultation and guidance services and promote their normative acceptability to PMTCT services and reduce the occurrence of HIV MTCT.
