Objective:To study the value of VTIQ technique in the diagnosis and evaluation of BI-RADS4 breast lesions,with a view to optimizing the diagnosis of BI-RADS4 breast lesions and providing more accurate and effective information for clinical treatment.Methods:102 breast lesions enrolled in the research were evaluated by conventional ultrasound for BI-RADS 4,and who would have biopsy or surgical treatment.Then into the VTIQ mode in order to measure the transverse shear wave velocity of tumor and the edge of the tumor,and the acreage ratio of tumorous with 2D ultrasonic imaging.The pathology results was the golden standard,all data processed software by SPSS-23.0.Draw the ROC curves of the SWV of tumor and the edge of the tumor and the AR in order to get the best diagnostic critical values.Finally,analysed the diagnostic sensitivity,specficy and accuracy of VTIQ for the BI-RADS 4 breast lesions.Results:There were total 102 breast lesions,of which 62 were malignant and 40 were benign.The differences between benign and malignant groups are statistically significant(P < 0.05).With the pathologic results as the gold standard,the ROC curves of the SWV of tumor and the edge of the tumor and the AR was drawn,and the best critical value of SWV of tumor which diagnosed benign and malignant is 3.85,whose diagnostic sensitivity,specficy and accuracy were 87.1%?72.5%?81.4%;The best critical value of SWV of the edge of the tumor which diagnosed benign and malignant is 2.13,whose diagnostic sensitivity,specficy and accuracy were 82.3%?70.0%?77.5%;The best critical value of AR which diagnosed benign and malignant is 1.89,whose diagnostic sensitivity,specficy and accuracy were 75.8%?77.5%?76.5%.And the area under the curve was 0.846?0.826 and 0.797,respectively.According to the evalution of SWV with 3.85m/s,33 cases of BI-RADS 4A breast lesions could be adjusted from 4A class down to 3 class.But there was 1 case of malignant tumor was misclassified as BI-RADS 3class,increasing the false negative rate.Conclusions:The study proved that VTIQ is helpful in diagnosis and evaluation of BI-RADS 4 breast lesions.It has a high diagnostic efficiency,and can reduce the needless puncture or operation of most BI-RADS 4A lesions,and provide a basis for the choice of clinical diagnosis and treatment.
