As a country with huge population and serious aging problem which lead to quite much lower economy per capita,China enjoys very much limited healthcare resources which play as an integral part of public medical treatment,causing extremely serious problems of difficult and expensive medical treatment.This requires the hospital to take proactive measures to optimize the medical resources so as to reduce medical costs and ease economic and mental pressure of both patients and their families to avoid causing poverty as much as possible.The functions of most medical resources of a hospital relies on the beds,which bear central functions of medical service.It is a highlight of medical management home and abroad how to improve utilization efficiency of beds to optimize medical resources through scientific management.At present,there are quite many indicators reflecting the allocation and utilization of beds including number of beds,turnover rate of beds,average hospital stay,bed utilization efficiency,etc.Among those indicators,average hospital stay and bed utilization efficiency can largely reflect the hospital’s managing ability,work quality and coordination among departments.Therefore,they are taken as comprehensive indicator for performance and widely applied among hospitals of various sizes and levels.At present,major domestic hospitals are exploring suitable managing models such as refined manage,target management,clinical path management,prepayment system(PPS),disease-based payment guidance(DRGs)and so on to reduce average hospital stay and optimize bed utilization efficiency so that bed resources can play a best medical value.In this study,a variety of influential factors for optimizing bed utilization efficiency are discussed.Based on literature analysis,managing measures and strategies for such optimization home and abroad are summerized so that relevant academic progress can be understood.Data is collected by case review of bed resources utilization between 2012 and 2016,including patient number at clinic,number of patients at hospital stay,average days of hospital stay,bed utilization rate,average days for hospital stay before surgeries,coverage of medicine,coverage of medical materials for departments and the hospital,etc.Based on charts,figures and data related,integrating interviews of experts and collection of medical treatment documents,present managing measures and effectiveness of JD Hospital are studied in practice,and comprehensive influence of measures related on average hospital stay and bed utilization rate is discussed.According to researches,the present refined management,target management and clinical path management of JD Hospital manage to reduce average hospital stay and optimize bed utilization rate to some degree,but are yet to be improved on some problems which need urgent solutions including gaps between practical and target quantitative management of clinical departments of surgeries and high bed utilization rate in departments accommodating a large number of patients.Referring to managing experience of major hospitals overseas,further systematic analysis to controllable and uncontrollable factors influencing average hospital stay and bed utilization rate is conducted in this study to put forward solutions as follows,including launching daytime ward system,providing hospice wards,rational allocation of beds,improving clinical paths,setting DRGs and dual transfer of treatment.These solutions aim to solve the bottlenecks and limitations brought by present measures in JD Hospital and seek more eligible solutions to optimize bed utilization efficiency,improve utilization quality of medical resources and provide theoretical support and guidance for medical treatment reform.
