Objective:Acute myocardial infarction(AMI)is a type of disease with high mortality and morbidity worldwide.At present,the diagnosis markers for cardiac infarction such as c Tn and ck-mb are mostly used,but they can be affected by other diseases(heart failure,inflammation,cachexia,peripheral organ infarction etc).Therefore,there are still of clinical significance to identify biomarkers for acute myocardial infarction.Micro-RNAs(mi R-RNAs mi RNAs)are a class of conserved short-chain non-coding RNAs,some of which involved in the regulation of the development of cardiovascular diseases.Micro RNAs are also altered and used as a potential marker of AMI due to their properties of stability and easy measurement.Some studies have showed that mi R-542-5p participated in oxidative stress and senescence and remodeling of cardiomyocytes,which played a role in the cardiovascular disease.Consistently,circulating mi R-542-5p was over-expressed in patients with myocardial infarction by microarray assay,but its predictive power for AMI patients is still need further studies.In this study,we aimed to investigate the plasma level of mi R-542-5p and analyze the predictive power for acute myocardial infarction.In addition,we analyzed gene database and found that mi R-542-5p specifically targeted at heat shock protein B7(HSPB7),which is a cardiovascular system-specific protein.To investigate the mechanism of mi R-542-5p,we also tested the plasma level of HSPB7 and assayed the correlation of mi R-542-5p and HSPB7.Furthermore,we also analyzed the predictive potential of plasma HSPB7 in AMI.Method:We enrolled 77 patients with AMI and 47 healthy subjects at Department of Cardiology,the Second Hospital of Jilin University from October 2017 to January 2018.We measured levels of plasma mi R-542-5p and HSPB7 in patients with AMI by real-time polymerase chain reaction and ELISA,respectively.Furthermore,we assayed the relationship between plasma mi R-542-5p and HSPB7 using the Spearman test.Moreover,we evaluated predictive powers of circulating mi R-542-5p and HSPB7 for patients with AMI using Receiver operating characteristic(ROC)curves.Results:1.In this study,we tested the peripheral plasma levels of mi R-542-5p in AMI patients and healthy subjects and the plasma levels of mi R-542-5p were significantly increased in AMI patients(13.05±12.33 fold changes)compared with the control group(P <0.01).2.We also measured the peripheral plasma levels of HSPB7 in AMI patients and healthy subjects.The result showed that plasma level of HSPB7 was increased in AMI patients(76.80±40.99ng/ml),compared with control(60.97±37.08ng/ml)(p< 0.05).3.We further assayed the correlation of mi R-542-5p and HSPB7.The results showed that mi R-542-5p positively correlated with HSPB7(r=0.237,p<0.01).4.We finally performed ROC analysis to evaluate the predictive potentials of circulating mi R-542-5p or HSPB7 for AMI.The AUC for mi R-542-5p was 0.935(95%CI:0.895-0.975;p<0.01),and the optimal cut-off value was 1.755 with sensitivity and specificity of 89.6% and 83%,respectively.The AUC for HSPB7 was 0.615(95%CI:0.514-0.716;p<0.05),and the optimal cut-off value was 52.453 with sensitivity and specificity of 63.6% and 51.1% respectively.5.We further analyzed the expression of mi R-542-5p in the AMI patients at different time points compared with the control group.The results showed that plasma mi R-542-5p of multiple infarction within 4 hours after AMI change was significantly higher than the control group,p <0.05.Meanwhile,plasma mi R-542-5p in AMI patients at 4-6,6-24,and >24 hour were significantly increased,compared with the control,p<0.05.Conclusion:1.Micro RNA 542-5p was significantly increased in the plasma of AMI patients,and HSPB7 was significantly increased compared with the control group,and micro RNA-542-5p positively correlated with HSPB7 expression.2.Plasma micro RNA-542-5p may be used as a biomarker for AMI.
