Cerebrovascular occlusion as an independent and important risk factor for ischemic cerebrovascular disease is an important concern of current treatments.Ischemic cerebrovascular disease area of ischemic infarct size,severity of clinical symptoms and prognosis and cerebral perfusion are closely related.Perfusion of brain tissue is consupplied by the anterior and lateral blood flow of the narrow feeding artery.In order to study the relationship between collateral blood flow and brain perfusion,we select patients with unilateral occlusion of middle cerebral artery.The cerebral perfusion in ischemic area is closely related to the formation of collateral circulation.The current study is limited to the type of collateral circulation,and limited to qualitative research,if we can collateral circulation hemodynamic changes combined with cerebral perfusion index,more conducive to the study of infarction mechanism and the choice of treatment options.Objective:The hemodynamic assessment of unilateral cerebral artery stenosis with cerebral arteriography(DSA)combined with CT cerebral perfusion imaging was used to analyze the relationship between collateral circulation and cerebral perfusion in the unilateral occlusion.The influence of baseline data was analyzed by statistical analysis Relevant factors of formation of pia mater collateral circulation.Method:Thirty patients with middle cerebral artery occlusion diagnosed by DSA were selected.Patients with middle cerebral artery occlusion were divided into good collateral circulation group and poor collateral circulation group according to the result of DSA.According to the CTP examination,the occlusion and mirror side CBV and CBF were collected,MTT,TTP,and rCBV,rCBF,rMTT and rTTP on the occlusion side and the mirror side,respectively.At the same time,the patients were divided into two groups: the cerebral perfusion group and the perfusion group.The differences of perfusion of brain tissue,the difference of perfusion of cerebral collateral circulation between benign collateral circulation group and collateral dysfunction group were analyzed.The related factors affecting the formation of collateral of cerebral pia mater were analyzed according to the baseline data.Result:There were no significant differences in CBV,CBF,MTT and TTP between affected side and contralateral side,CBV,CBF and MTT in affected side and contralateral side,and TTP in contralateral side and healthy contralateral side(P <0.05)There is statistical significance.There was significant difference between rCBV and rCBF in benign collateral circulation group and poor collateral circulation group(P <0.05).Collateral circulation and cerebral perfusion were positively correlated.Low density lipoprotein(LDL)in collateral circulation was good and poor group difference was statistically significant(P <0.05).Conclusion:There were differences in perfusion on both sides of middle cerebral artery occlusion patients.The formation of collateral circulation was closely related to cerebral perfusion.High LDL may promote the formation of collateral circulation.The combination of DSA and CTP can provide a comprehensive and comprehensive evaluation of patients with unilateral middle cerebral artery occlusion and thus provide the basis for the selection of the best treatment and treatment options.
