Objective: This experiment is mainly studied in the following aspects: 1)The characteristics of brucellosis spondylitis in X-ray,CT and MRI scan were summarized.2)To investigate the changes of ADC and FA values in each period of brucellosis spondylitis by diffusion tensor imaging.3)To investigate the changes of ADC and FA values of DTI before and after conservative treatment.4)The correlation between the titer of DTI and the serum agglutination test of the disease was further analyzed.Methods: CT,MRI and conventional spiral DTI scan were performed in our hospital and the cooperation unit admissions of 37 brucellosis patients and 37 normal controls were selected for vertebral spine,all ADC and FA measurements in acute stage(10),subacute stage(13),chronic phase(14),normal control group ADC and FA value of the single factor analysis of variance;the follow-up after conservative treatment of vertebral lesions by ADC and FA,the two groups before and after treatment by paired t test numerical;the measured all brucellosis patients by ADC and FA respectively,and the correlation between the titer of serum agglutination test.Results: showed that CT found a total of 65 vertebral bodies,and 99 cases were found by routine MRI.The number of diseased vertebral bodies in routine MRI was significantly better than that of CT.The acute,subacute,chronic and ADC values of the control group were analyzed by variance,the difference was statistically significant(F=270.04,P=0.000),and the difference in FA values was statistically significant(F=7.12,P=0.000).There were no statistically significant differences in ADC between acute and subacute phase(P values was 0.714).The ADC values in acute,subacute and chronic phases were significantly higher than those in the control group,and the difference was statistically significant(P values were 0.000,0.000,0.000);The ADC value of chronic phase was lower than that of acute and subacute,and the difference was statistically significant(P values were 0.024,0.043).There was no statistical difference between the FA value of the control group the acute and subacute phases(P value was 0.958,0.062,0.076);The FA value in chronic phase was lower than that in control group,acute phase and subacute phase,and the difference was statistically significant(P value was0.007,0.015,0.000).The value of ADC in the acute,subacute and chronic stages was lower than that before the treatment,and the difference was statistically significant(respectively,and the P values were 0.002,0.010,0.001).Acute and subacute phase after treatment FA value was lower than that before treatment,the difference was statistically significant(P values were 0.002,0.013),chronic phase FA value after treatment than those before treatment(P=0.043).The ADC value was positively correlated with the titer of serum agglutination test,with statistical significance(r=0.837,P=0.007);FA value was zero correlated with serum agglutination test titer(r=-0.197,P=0.16).Conclusion: 1)MRI routine scan is superior to CT and X ray.It can detect the affected vertebral body before the shape of vertebral body changes.2)DTI examination can quantify the BS vertebral changes in each stage by measuring the FA value and ADC value,and roughly guide the BS stage,so as to guide the clinical treatment,and further provide a reliable index for evaluating the prognosis of BS.3)Changes in FA and ADC values before and after treatment can be used to evaluate the prognosis.4)By analyzing the relationship between FA and ADC value and the titer of serum agglutination test,we can evaluate the vertebral involvement and reduce the economic burden of patients in the later stage.
