Objective:This study aims to detect the state of existence of HPV16-DNA in the cervical exfoliated cells of different cervical lesions,and analyse the relationship between them,At the same time,it is going to explire the value of the integrated state of HPV16-DNAin predicting and diagnosing the high level cervical lesions,in the early sureening of cervical cancer,so as to fing a simple,non-invasive and valid methed of the screening of cervical cancer.Methods and objects:1.Choose 124 patients who have got positive HPV16 and biopsy of the cervical from May 2017 to November 2017 of the gynaecology in the first affiliated hospital of the university of south China.According to the pathological diagnosis of cervical biopsy,the patients are devided into four groups:chronic cervicitis,low-grade cervical lesions,high-grade cervical lesions and cervical cancer,the age of patients varied from 18 to 73 years old,the average age is 45.5 years,the inclusion criteria: All the patients were non-pregnant,have no history of any treatment about the cervix,no other sexually transmitted disease,no history of radiotherapy and chemotherapy of the pelvic disease;2.Take the cryopreserved sample of cervical exfoliated cells of the research objects,detemine the gene of E2 and E6of HPV16-DNAwith PCR technology and detemine the ratio of E2/E6 with agarose gel electrophoresis.and then determine the state of existence of HPV16-DNA according to the ratio of E2/E6,analyse the state of exisrence of HPV16 DNA in different level of cervical lesions,and their correlation.Results:(1)The state of existence of HPV16-DNAof the cervical exfoliated cells of different cervical lesions is devided into three groups:the free type,mixed type,integrated type,their corresponding number are 26,35 and 63,the percentage are 21%,28% and 51%;(2)The state of existence of HPV16-DNA of cervical exfoliated cells of different levels of cervical lesions is different,the experiment results show that:The percentage of the free type in chromic cercicitis,low-grade cervical lesion,high-grade cervical lesion and cervical cancer accounted for 76.9% ? 55.6% ? 16.7% and 4.17%,respectiveely the percentage of the mixed type accounted for 15.4%?33.3%?38.9% and 18.75%;the percentage of the integrated type accounted for7.7%?11.1%?44.4% and 77.08%.The percentage of HPV16-DNA in different levels of cervical lesions was different,and the difference was statistically significant(x2=50,p <0.05).And the integrated type of the cervical exfoliated cells,from the patients with HPV16 becomes higher when the level of cervical lesions become serious,while the free type becomes lower,It was siatistically analyzed that The integration degree of HPV16-DNA in the cervical exfoliated cells had a positive correlation to the severity of the cervical lesions(r=0.91,p<0.05);(3)The sensitivity of the rate of existance of HPV16-DNA in diagnosing of high-grade cervical lesions and cervical cancer is 59.8%,the specifity is 90.9%,the positive predictive value was 96.8%,the negative predictive value of 32.8%.Conclusion:1.The integration degree of HPV16-DNA in cervical exfoliated cells had a postive correlation to the severity of the cervical lesions,so the integtation of HPV16-DNA in cervical exfoliated cells play an important role in the progression of cervical cancer;2.The detection of the state of integration of HPV16-DNA in cervical exfoliated cells is simple and non-invasive,it has high specificity and positive predictive value in the screening for cervical.
