The Comparative Research Between Femoral Stem Prosthesis With Fixed Neck Length And Varying Neck Length On Reconstruction Of Hip In Total Hip Arthroplasty

Objective:This study summarized and compared the clinical data of patients who underwe-nt Total Hip Arthroplasty by femoral prosthesis with different neck length,so as to discuss the reconstruction results of hip,to find more clinical practice for choice of prosthesis in Total Hip Arthroplasty in Chinese.Methods:Between February 2017 and February 2018,83 patients who were consecutively treated in the orthopedic joint group and were performed by the same group of physic-cians and were undergoing unilateral Total Hip Arthroplasty accorded with selection criteria were selected for a retrospective study in The First Affiliated Hospital of Kun-ming Medical University.The patients were divided into two groups:during Total Hip Arthroplasty,femoral prosthesis with the Stryker Accolade TMZF were used in 54 patients(Group A)and prosthesis with the Link Classic Uncemented were used in 29 patients(Group B).The postoperative Harris score was recorded to evaluate the clin-ical outcomes after Total Hip Arthroplasty at 3 months,and the femoral offset of con-tralateral and operated side、the leg discrepancy of contralateral and operated side、the femoral calcar of operated side were measured.Data were collected for statistical ana-lysis and the statistical analysis was performed using the SPSS21.0.The quantitative data were expressed as mean ± standard deviation,and whether the sample K-S(1)analysis data was normally distri-buted,two independent sample t-tests were used for the comparison between normal distribution groups,whereas non-parametric tests were performed on the data.The qualitative data were compared with the χ2 test,the test levels were a=0.05,and there was a significant difference(P<0.05).Results:According to the imaging data,the femoral offset of contralateral and operated side、the leg discrepancy of contralateral and operated side、the femoral calcar of operated side in the A and B groups were determined.The related data were compared and the following results were obtained.In addition to the height,there were no significant difference in gender、age、weight、BMI between two groups(P>0.05).There was a significant difference between the femoral offset of contralateral and operated side in patients A and B(P=0.001).The difference in femoral offset between patients in group A was within ±4 mm(can be considered’ as reconstruction of femoral offset),and in 96.29%of patients,the femoral offset difference was concentrated within ±2mm.In group B,55.17%of patients had femoral offset difference within±4mm.The mean of the femoral offset difference in group A was less than 4 mm(0.82 mm),and the mean of the femoral offset difference in group B was greater than 4 mm(4.09 mm).There was a significant difference between the leg discrepancy of contralateral and operated side in patients A and B(P=0.001).In group A,42.6%of the patients had a leg discrepancy within ±5 mm(can be considered as equal of the leg discrepa-ncy).In group B,20.68%of patients had a leg discrepancy within ±5mm.The mean of the leg discrepancy of the group A was slightly greater than 5mm(5.56mm),and the mean of the leg discrepancy of the group B was close to 10mm(9.09mm).There was a significant difference in the femoral calcar of operated side between the two groups(P=0.02).In group A,85.19%of patients had preserved femoral calcar of operated side greater than 10mm(can be considered as not having been damaged).In group B,72.41%of the patients had preserved femoral calcar of operated side grea-ter than 10mm.The mean of the femoral calcar of operated side between the two gro-ups was greater than 10mm,but the mean of group A(13.15mm)was greater than group B(11.02mm).The postoperative Harris score was recorded to evaluate the clinical outcomes after Total Hip Arthroplasty at 3 months,there was a significant difference(P=0.001).In group A,35.19%of patients of Harris score was excellent,and 64.81%was good.In group B,68.97%of patients of Harris score was good and 31.03%was moderate.The mean of Harris scores of patients A and B were between 80-90(good),but the mean of group A(89.33)was greater than group B(81.87).Conclusions:1.For the population in the southwest region,the femoral prosthesis of the Stryker Accolade TMZF with a non-single-neck length in the total hip arthroplasty.The reconstruction results of the femoral offset of operated side,the protection results of the femoral calcar of operated side and the maintaining results of the leg length of both lower extremities were better than the femoral prosthesis of the Link Classic Uncemented with single-neck length.And for the femoral prosthesis of the Stryker Accolade TMZF,the reconstruction results of the hip of operated side was better than the femoral prosthesis of the Link Classic Uncemented s.2.For the femoral prosthesis of the Stryker Accolade TMZF with a non-single-neck length,the recent clinical efficacy reflected by the Harris score was better than the femoral prosthesis of the Link Classic Uncemented with single-neck length in Total Hip Arthroplasty.
