Study On Correlation Between LVEF And NT-proBNP Level In Hospitalized Patients With Heart Failure And Preserved Ejection Fraction

Objective To investigate the correlation between the left ventricular ejection fraction(LVEF)and N terminal brain natriuretic peptide precursor(NT-pro BNP)level in the inpatients with heart failure(HF)and preserved ejection fraction(HFpEF).Methods A total of 339 hospitalized patients with chronic heart failure were collected and grouped according to the Heart Failure Guidelines of the European Society of Cardiology in the Department of Cardiology of Second Affiliated Hospital of Xi?an Medical University from December 2016 to December 2017.Among them,there were 159 males and 180 females,aged 33 to 92(71.43±11.61 years),and 201 patients with coronary atherosclerotic heart disease,193 patients with hypertension,51 patients with arrhythmia,35 patients with type 2 diabetes,52 patients with chronic gastritis,32 patients with anemia,6 patients with hyperlipidemia,42 patients with old cerebral infarction,old cerebral hemorrhage There were 1 patient,18 patients with chronic bronchitis,1 patient with chronic renal failure,8 patients with cervical spondylosis,and 4 patients with lumbar disc herniation.The NT-pro BNP level,biochemical indicators and cardiac Doppler ultrasonic parameters were detected.Then the correlation analysis of detection results was performed.Results Significant statistical differences between NT-pro BNP in patients with HFpEF,HFmr EF,and HFr EF(t=17.16,P<0.001),and in all patients with heart failure,there was a significant negative correlation between LVEF and serum NT-pro BNP levels(R=-0.470,P<0.001).The correlation between NT-pro BNP and LVEF was not statistically significant in patients with HFpEF(R=-0.116,P>0.05).There is a significant negative correlation between serum NT-pro BNP and LVEF in patients with HFmr EF(R=-0.353,P<0.05),and this significant negative correlation between serum NT-pro BNP and LVEF in patients with HFr EF(R=-0.367,P<0.05).The ROC curve was plotted with NT-pro BNP in HFpEF patients.The area under the curve was calculated as 0.827,P<0.05.By calculating the Youden index,the best specificity and sensitivity were obtained at NT-pro BNP=1210.64 pg/ml.The sensitivity was 90.3% and the specificity was 56.9%.Significant positive correlation between low-density lipoprotein and LVEF in patients with HFpEF(R=0.159,P<0.05).Conclusion In heart failure patients,serum NT-pro BNP levels increase as LVEF levels decrease,but this pattern does not follow in patients with HFpEF.However,sensitivity was 90.3% and specificity was 56.9% when HFpEF was differentiated by taking NT-pro BNP=1210.64pg/ml as the critical value.That is,in patients with HFpEF,90.3% of patients had serum NT-pro BNP >1210.64 pg/ml;conversely,among patients with NT-pro BNP >1210.64 pg/ml,56.9% of patients were diagnosed with HFpEF.
