| Objectives 1 We explore the influence of comprehensive evaluation and individualized intervention on wound healing of diabetic patients with chronic wound;2 We explore the influence of comprehensive evaluation and individualized intervention on the physical and mental health of patients with diabetes mellitus with chronic wound.Methods From September 2016 to September 2017,88 cases of type 2 diabetes mellitus with chronic wound patients were recurited from two tertiary grade hospitals in tangshan district.The subjects were randomly divided into two groups according to the score of wound score,and 44 cases in the intervention group and the control group respectively.The patients of the two groups were treated with wet healing therapy based on local assessment of wound.The control group received routine nursing measures except wound treatment.The intervention group conducted comprehensive and comprehensive assessment on the basis of the control group,including the physical and mental condition,the health problems in the evaluation,and the individualized prescription intervention.Before and after the intervention,the rehabilitation effects of chronic wounds were evaluated in the two groups,and the indicators of other physical and mental conditions were evaluated,and the differences were compared.SPSS17.0 was used for statistical analysis.Continuous variables were presented as mean ± standard deviation(SD)and were compared by ANOV As.Categorical variables were described as percentages and were compared using chi-square tests(x~2 tests).Results 1 Local wound healing After 4 and 8 weeks intervention,area size of the wound,pain scores and PUSH scores of the intervention group is better than the control group(P<0.05);The number of days of wound healing in the intervention group was less than that in the control group at different stages(P<0.05).2 Two groups of patients were compared in clinical metabolic indicators After 4 weeks of intervention of serum albumin(ALB)and hemoglobin(HGB),the value of the intervention group was significantly higher than that in the control group(P<0.05);After 4 weeks of intervention of fasting blood glucose(FBG),blood glucose in the intervention group was significantly lower than that in the control group(P<0.05);Hb A1 c was compared in the three months after intervention,and the value of the intervention group was significantly lower than that in the control group(P<0.05),and it was close to normal level.3 The SAS scores were compared between the two groups There was no statistical difference in anxiety scores before intervention(P>0.05).After 4 weeks of intervention,the score of the intervention group was significantly lower than that in the control group,and the intervention group was more effective in alleviating anxiety,and there was a statistically significant difference between the two group and the group(P<0.05).4 We compared the social support scale between the two groups After 4 weeks of intervention,the scores in the intervention group were higher than those in the control group,with statistically significant differences(P<0.05).There was no significant difference in the comparison results between the control group and the group(P>0.05),and the social support of the intervention group was better than that of the control group.5 The self-management status of the two groups was analyzed Intervention in the intervention group patients after 4 weeks of self management dimension scores are higher than the control group,two groups was statistically difference(P<0.05),showed that after intervention,the intervention group self-management ability level is better than the control group.6 The life quality of the two groups After 4 weeks intervention,the each dimension score of the life qualityof the intervention group was obviously improved than the control group,with statistical significance(P<0.01 or P<0.05),which effectively improve the patients’ life quality.Conclusions Comprehensive evaluation and individualized intervention can effectively promote the healing of diabetic patients with chronic wounds,improve the patients’ mental health,promote the patients’ self management level and the level of social support and increase the life quality. |
