| OBJECTIVE: To investigate the association between hypertension and the positive test of treadmill exercise test in patients with an isolated descending myocardial bridge.METHODS: From January 2011 to June 2016,8104 patients who had undergone CAG examination in the Department of Cardiology,Hangzhou Normal University Affiliated Hospital for chest discomfort,chest pain,palpitations,and other discomforts were selected.Among them,941 patients with myocardial bridge were found.A total of 305 patients with isolated myocardial bridges(without coronary atherosclerosis and other heart diseases)who were only located in the anterior descending coronary artery were screened for the study.They did not perform treadmill exercise tests before CAG and did not meet the study criteria.In patients undergoing treadmill exercise,only 223 eligible patients were left.According to the results of the TET test,the patients were divided into TET positive group(115 cases)and TET negative group(108 cases).Univariate and multivariate Logistic model was used to analyze the difference of hypertension between the two groups,and then based on the patient’s coronary artery contraction.The degree of stenosis was grouped into ≤30%,30-50%,50-75%,≥75%.Using single factor and multivariate logistic models to compare the differences in hypertension between subgroups.Results:Through univariate analysis and multivariate logistic regression analysis of TET positive and negative groups,it was suggested that hypertension(P=0.002 OR=2.577 OR 95% CI was 1.427-4.653),and the degree of systolic stenosis of the coronary artery was ≥75%(P=0.023 OR=2.602 OR 95% CI was 1.040-5.941)was associated with a positive TET test.The comparative analysis of TET positive and negative groups in the systolic phase stenosis group of patients with different myocardial bridges suggests that the systolic stenosis of the patients with isolated coronary artery in isolated myocardial bridges is less than or equal to 30%,hypertension,and TET.Positive correlation(P=0.035,OR=2.780,OR 95% CI was 1.073-7.203),and when systolic stenosis was 30-50%,hypertension was associated with TET positive(P=0.017,OR=3.564).OR 95% CI was 1.261-10.073).When the degree of systolicstenosis of the patient’s coronary arteries was between 50-75% and ≥75%,there was no significant correlation between hypertension and TET test positive(P>0.05).CONCLUSIONS: Hypertension and systolic stenosis of the coronary artery≥75%are associated with positive treadmill exercise test in patients with isolated myocardial bridge of anterior descending coronary artery.At 50%,hypertension is still associated with positive treadmill exercise tests,which may suggest that patients with myocardial bridges with hypertension are more likely to undergo positive changes in treadmill exercise tests than those without hypertension.Myocardial ischemia is more likely to occur. |
