Study On The Correlation Between Pathologicl Staging Of Liver And Antiviral The Rapy

Objectives In this study,patients with chronic hepatitis B(HBV)were examined by liver biopsy to understand the liver cell inflammation,fibrosis and serological examination.The relationship between them was analyzed.The relationship between the best time of antiviral treatment and the effect of antiviral treatment was grasps.The suitable diagnostic method is a new idea for the diagnosis of chronic hepatitis B,which is not in accordance with the guide for the prevention and treatment of chronic hepatitis B,but the time of antiviral treatment,the effect of antiviral treatment and the diagnosis of abnormal liver function patients.Methods The subjects were 90 patients with chronic hepatitis B who met the requirements of Pathology Department of Qinhuangdao Third Hospital in September 2015-2017 November.There were 70 cases in the treatment group and 20 cases in the control group,ranging in age from 26 to 62 years old.The diagnosis of hepatitis B virus infection should be in accordance with the guidelines for the prevention and treatment of chronic hepatitis B in 2015.12 cases of hepatitis B virus infection,16 cases of fatty liver and 8 cases of drug-induced hepatitis were selected as positive controls.Liver fibrosis and liver function examination were performed in 2 days before and after the examination of liver biopsy.The abdominal ultrasound and liver noninvasive fibrosis(FibroScan)were required before the biopsy of the liver.The liver structure and the degree of liver fibrosis were understood.Under ultrasound guidance,a suitable puncture site was selected and liver puncture was performed at the location point,and guided by ultrasound.The results were compared with the pathological staging of liver biopsy,according to the relationship between the pathological results of liver biopsy and the degree of liver fibrosis,and the significance of the serological examination for the degree of liver fibrosis was found.Results The correlation between the degree of hepatocyte inflammation and the degree of liver fibrosis is obtained through the biopsy of liver biopsy,and it is of guiding significance to the treatment of the virus.The patients can be treated in time according to the pathological staging of liver puncture,which conforms to the guide for the prevention and treatment of chronic hepatitis B.After 24 weeks of antiviral treatment,the liver biopsy is performed again,and the changes are compared before and after the treatment.After the complete response,the degree of inflammation or fibrosis of hepatocytes in the complete response group improved(P<0.01).The degree of inflammation of hepatocytes in the partial response group was partially improved(P<0.05),but the degree of fibrosis was not significantly improved(P>0.05),but there was no significant change in the degree of inflammation and fibrosis in the non response group(P> 0.05).ALB,GLO,HA,PCIII,CIV,ALT,AST,GGT,TBIL,LN in the stages of different liver fibrosis were P <0.05,there were statistical differences.The levels of ALT,AST,GGT,TBIL,ALB and GLO were negatively correlated with the degree of liver fibrosis.Conclusions 1 nucleotides can be treated with antiviral therapy,the complete response to virology can significantly improve the degree of liver inflammation and fibrosis.2 The direct serological index,in the diagnosis of liver fibrosis,the coincidence rate of LN is low,PCIII and IV type collagen have certain guiding role,while HA has a higher guiding role,all indexes after antiviral treatment are obviously improved.3 There was no correlation between indirect serological indexes and liver fibrosis degree: ALT,AST,GGT and TBIL.After antiviral treatment,the level of aminotransferase was obviously decreased.The diagnostic significance of serum albumin and serum globulin level in the diagnosis of fibrosis and cirrhosis needs further study.4 The FibroScan examination showed that the value of liver hardness increased with the increase of liver fibrosis,the rate of coincidence in S4 phase was the highest,and the accuracy in the diagnosis of S1-S3 phase should be improved,and the liver elasticity of some patients after antiviral treatment decreased.
