Objectives The effect of Kegel exercise on the function of pelvic floor after 28 weeks of pregnancy in the late pregnancy,delivery and postpartum pelvic floor function.Methods Research object: Randomly selected at the beginning of November 2016 to the end of August 2017 in Tangshan City North Polytechnic Affiliated Hospital of University of pregnant women,According to the inclusion criteria,exclusion criteria and voluntary principles,373 subjects were included in the study.They were randomly divided into the observation group and the control group,with 177 people in the observation group and 196 people in the control group.Research methods: The pregnant women in observation group were guided by special person for Kegel exercise after 28 weeks of pregnancy.The control group was tested for normal production and did not carry out the guidance of Kegel exercise.The changes in pelvic floor function of pregnant women in the late pregnancy and clinical data of patients during childbirth were collected by filling out the questionnaire,including age,pre pregnancy BMI,weight gain during pregnancy,gestational age,perinatal pelvic floor disease related symptoms,neonatal weight,duration of labor,perineal lateral incision,perineal tear,midwifery and other related clinical data.During 6 weeks postpartum review,the dynamic pressure of pelvic floor,I muscle fiber and II muscle fiber were detected by filling out questionnaire and French PHENIX USB4.Statistical method : SPSS17.0 statistical software was used for analyzing all the data.The measurement data was expressed in mean number and standard deviation;t test was used for comparison between groups;counting data was checked by chi square test;rank data was tested by rank sum test.The difference between P<0.05 was statistically significant,and P<0.01 has extremely statistically significant difference.Results 1 There was no significant difference in urinary incontinence and PFIQ-7 score between 28 weeks of pregnancy and 32 weeks of pregnancy in the observation group compared with the control group(P>0.05).The level of urinary incontinence and PFIQ-7 score in 36 weeks of pregnancy in observation group were lower than those in control group,and the difference was statistically significant(P<0.05).The difference between the two groups of PFIQ-7 scores at three time points was statistically significant(P < 0.05);the difference in PFIQ-7 score between the two groups was statistically significant(P < 0.05);the observation group,the control group and the time point had the interaction effect(P < 0.05),suggesting that the PFIQ-7 score was related to the number of gestational weeks.2 The level of urinary incontinence on the moment of delivering and PFIQ-7 score in observation group were lower than those in control group,and the difference was statistically significant(P<0.05).The rate of cesarean section in observation group was lower than control group(P<0.05).Compared the labor time of observation group and control group for vaginal delivery,the first stage,the second stage,the third stage and total labor time of observation group was less than control group.The first stage,the second stage and total labor time of observation group compared with control group,the difference was statistically significant(P<0.05).The labor time of third stage in observation group was less than control group,but the difference was not statistically significant(P>0.05).The perineum integrity rate in observation group was higher than control group.The proportion of perineum side cut was lower than control group,the difference was statistically significant(P<0.05).The proportion of I degree perineal tear in observation group was higher than control group.The proportion of II degree and III degree perineal tear in observation group was lower than control group.But the difference was not statistically significant(P>0.05).3 The level of urinary incontinence and PFIQ-7 score in observation group were lower than control group 6 weeks later after delivery,and the difference was statistically significant(P<0.05).Compared with the muscle strength of I and II class muscle fibers 6 weeks later after delivery in observation group and control group,the muscle strength of observation group was higher than control group,and the difference was statistically significant(P<0.05).Compared with the fatigue degree of I class muscle fiber and II class muscle fiber 6 weeks after delivery in observation group and control group,the fatigue degree of I type muscle fiber and II class muscle fiber in observation group were higher than control group,the difference was statistically significant(P<0.05).The dynamic pressure of pelvic floor muscle in observation group was higher than control group 6 weeks later after postpartum,and the difference was statistically significant(P<0.05).Conclusions 1 Kegel exercise in late pregnancy can reduce the related symptoms of pelvic floor dysfunction during pregnancy and postpartum.2 Kegel exercise in late pregnancy can reduce the rate of caesarean section,improve the perineum integrity,and shorten the time of production.3 Kegel exercise in late pregnancy can improve the muscle strength of postpartum pelvic floor muscle fibers,improve the fatigue of I type muscle fiber and II type muscle fiber,increase the dynamic pressure of pelvic floor,and reduce the incidence of pelvic floor dysfunction.
