| Objective:The aim of this study was to evaluate the effects of computer-based cognitive training on cognitive function, activities of daily living, quality of life and subjective well-being in elderly patients with mild cognitive impairment in China, and explore effective ways to postpone the transition from mild cognitive impairment to dementia. Methods: This study is an experimental study.According to inclusion and exclusion criteria,61 elderly patients with mild cognitive impairment in a nursing home in Hangzhou were selected as the research subjects, including 29 in the experimental group and 32 in the control group. The control group received the daily life care of the pension institution, and the experimental group received a 30 minutes computer-based cognitive training three times weekly on the basis of the daily life care of the pension institutions. The intervention form of the experimental group was group intervention, and the total time of intervention was 12 weeks. The Montreal Cognitive Assessment(MOCA),Activity of Daily Living Scale(ADL),World Health Organization Qulity of Life(WHOQL-BREF)and Memorial University of Newfoundland Scale of Happiness(MUNSH)were used to measure the cognitive function, ability of daily living, quality of life and subjective well-being respectively before and after intervention in both groups.Results:(1)Cognitive function. After intervention, the total score of MOCA,visuospatial and executive function, attention, language and delayed recall in the experimental group were higher than those before intervention, and these aspects were higher than those in the control group(P < 0.05).In the control group, there was no statistical significance(P > 0.05)in the total scores and the dimensions of MOCA before and after the intervention. It can be seen that computer-based cognitive function training can improve the cognitive function of the elderly patients with the mild impairment, especially in the visual space and executive function, attention, language and delayed recall.(2)The ability of daily life. After intervention, the total score of ADL,telephone, medication, disposal of personal belongings and instrumental activities of daily living in the experimental group were lower than those before intervention, and these aspects were lower than those in the control group(P < 0.05).In the control group, there was no statistical significance(P > 0.05)in the total scores and the dimensions of ADL before and after the intervention. It can be seen that computer-based cognitive function training can improve the daily living ability of the elderly patients with mild cognitive impairment, especially in terms of instrumental activities of daily living, such as phoning, take medicine, handle money.(3)Quality of life. After intervention, the mental health, social relations, overall quality of life and health in the experimental group were higher than those before intervention, and these aspects were higher than those in the control group(P<0.05).In the control group, there was no statistical significance in the quality of life score and its dimensions before and after the intervention(P > 0.05).It can be seen that computer-based cognitive function training can improve the quality of life of elderly patients with mild cognitive impairment, especially mental health, social relations, overall quality of life and health.(4)Subjective well-being.Before and after intervention, there was a statistically significant difference in total score of MUNSH,positive emotion, negative emotion, positive experience and negative experience. The scores of MUNSH,positive emotion, positive experience after intervention were higher than that before the intervention. In the control group, there was no statistical significance in the total score of subjective well-being and its dimensions before and after the intervention(P > 0.05).The two groups had statistical significance in the MUNSH total score, positive emotion, positive experience, negative emotion and negative experience after intervention(P < 0.05).The score of the experimental group in MUNSH,positive emotion, positive experience after the intervention was higher than that of control group, and the score of the experimental group in negative emotion and negative experience was lower than that of control group. It can be seen that computer-based cognitive function training can improve the subjective well-being of the elderly patients with mild cognitive impairment, increase the positive emotion and positive experience of the elderly, and reduce the negative emotion and negative experience of the elderly. Conclusions: Computer-based cognitive function training can effectively improve the cognitive function, activities of daily living, quality of life and subjective well-being of the elderly patients of mild cognitive impairment, and it can carry out the popularization and application in the community as a rehabilitation nursing measure which delay the elderly with mild cognitive impairment to dementia in the community. |
