| Objectives: By comparing and analyzing the imaging features of Legionella pneumonia(LP)and Mycoplasma pneumonia(MP),find out the imaging features of Legionella pneumonia and improve the understanding of its imaging features,so as to improve the level of diagnosis and differential diagnosis of Legionella pneumonia,achieve early diagnosis and treatment,reduce mortality and improve the prognosis.Methods: We retrospected of patients from January 2012 to December 2017.In present study a total of 50 patients diagnosed Legionella community-acquired pneumonia and 50 patients diagnosed Mycoplasma community-acquired pneumonia were included.The imaging features of them were collected.Results: In the type of lesions,the main manifestations of LP group were consolidation with grinding glass shadow(GGS),which is patch shadow and consolidation in MP group.Consolidation with a clear boundary in the glass-grinding shadow was only exist in LP group.The difference was statistically significant.There were both large lobar distribution and distribution along bronchi in LP and MP group.However,the main features of LP group were large lobular distribution,while that of MP group was mainly along the bronchus or both.The imaging distribution of LP group was mostly bilateral lesion,and that of MP group was mostly unilateral lesion.The differences have statistical significance.In accompanying signs,In LP group,the enlargement of hilar or mediastinal lymph nodes and pleural effusion were more than those in MP group.There were 10 cases of reverse halo sign only in LP group,and 30 cases of tree-bud sign only in MP group.The results of two differences have statistical significance.Conclusions: In this study,the imaging features of Legionella pneumoniae were found to be bilateral lesions and lobular distribution,the lesions were mainly solid and GGS,and most of them were accompanied by pleural effusion.Enlargement of hilar or mediastinum lymph nodes,honeycomb changes,cavity,pleural changes and bronchoinflatable sign were found.The imaging features of Mycoplasma pneumoniae include patch shadow and consolidation,GGS and nodular shadow,and unilateral lesion,which may involve one or more lobes.Only a few patients with a single lesion more than half lobe,pleural effusion or enlargement of hilar or mediastinum lymph nodes,and tree-bud sign could be used to differentiate Legionella pneumonia from Mycoplasma pneumonia. |
