Primary Study Of Acetabular Endoplasty In Developmental Dislocation Of The Hip

Objective: Through the evaluation of the imaging indexes after the development of acetabulum endoplasty for developmental dislocation of the hip,the postoperative radiological evaluation is carried out to provide further basis for its popularization and application.Methods: Retrospective analysis of the development of hip dislocation and simple open reduction combined with Dega pelvis from January 1,2014 to December 31,2016 in our department acetabular endoplasty combined with Dega pelvic osteotomy Development of osteotomy for the development of hip dislocation.Collect medical records and pelvis radiographs for the last follow-up.The Acetabular index,Acetabular-Head index,width of teardrop and Center-head distance discrepancy were measured on X-ray.The necrosis of femoral head was determined according to Kalamchi-Mac Ewen classification.The data obtained using SPSS16.0 software for statistical analysis.Results: Thirty-eight(48 hips)of developmental dislocation of the hip treated with acetabular endoplasty combined with Dega pelvic osteotomy and 22(25 hips)of hip dislocation treated with simple open reduction and Dega pelvic osteotomy were performed between the two groups Age,follow-up time and preoperative Tonnis classification.There was no difference in femoral neck osteotomy(P> 0.05).AI 21.83 ± 6.51 ° in the acetabulum endoplasty group and AI 14.32 ± 6.60 ° in the open reduction group were significant(P <0.001).There was no significant difference(P=0.909)in AHI 74.00 ± 11.43% after acetabular endoplasty and 73.65 ± 13.96% in AHI alone.The width of teardrop was 6.06 ± 2.21 mm in the group of acetabulum endoplasty,and the width of teardrop was 6.39 ± 1.99 mm in the group of simple open reduction.There was no significant difference(P=0.538).The incidence of AVN in acetabular angioplasty group was 16.67%.The incidence of AVN in open acetabular reconstruction group was 12%,there was no significant difference(P=0.597).Acetabular endoplasty combined with Dega pelvic osteotomy for unilateral hip dislocation 17 hips and simple open reduction combined with Dega pelvic osteotomy for unilateral hip dislocation 28 hips between the two groups the operation age,follow-up time,surgery.There was no difference in the former Tonnis classification(P> 0.05).The CHDD in the acetabular endoplasty group was 6.80 ± 5.03%,and the simple open reduction group was 10.61 ± 5.82%.The difference was significant(P =0.031).Conclusion: Acetabular endoplasty without affecting acetabular development and necrosis of the femoral head can effectively reduce the CHDD value,improve the side of the femoral head offset,in order to obtain a better concentric reduction.
