Objective To investigate recurrent risk factors of ultrasound-guided primary liver cancer after microwave ablation(MWA).Methods 107 lesions of the 87 hepatic carcinoma cases were undergone ultrasound guided percutaneous MWA.The curative effect were observed,and calculated the recurrence rate following up 5 ~ 73 months.Kaplan—Meier method was used to calculate disease-free survival time of patients.Chi-square test was used to compare the risk factors for recurrence of tumor after MWA.Results The whole recurrence rate of tumor was 45.98%.There were 92 lesions in the diameter of the tumor < 3 cm,36 lesions were recurred.There were 15 lesions in the diameter of the tumor ?3 cm,10 lesions were recurred,P=0.046(P<0.05,the difference was statistically significant).Among the 71 patients whose tumor was single,there were31 patients relapsed,9 cases were relapsed of 16 multiple patients,P=0.36(P> 0.05,the difference was not statistically significant).The recurrence rates were respectively37.50%?33.33%?37.50%?66.67%?100.00%,when the lesions located in the normal position ? near the diaphragm,the large blood vessel,the surface of the liver and the gallbladder.The median disease-free survival time of the patients in the relapse group was 12 months,and the median disease-free survival time of the patients in the non recurrent group was 19 months.The recurrence rate of well-differentiated hepatocellular carcinoma was 57.14%,the recurrence rate of moderately-differentiated hepatocellular carcinoma was 63.16%,P=0.727(P>0.05,the difference was not statistically significant).The median disease-free survival time in well-differentiated hepatocellular carcinoma was 17 months,and the median disease-free survival time in moderately-differentiated hepatocellular carcinoma was 12 months.Conclusion The diameter of the tumor and tumors locating near the liver surface or the gallbladder will increase the risk of recurrence of liver cancer after microwave ablation.Although the degree of tumor differentiation does not have a direct impact on the risk of recurrence after the operation,however,well-differentiated hepatocellular carcinoma has a longer disease-free survival time than mid-differentiated hepatocellular carcinoma.
