Objective: To analysis the effect of pelvimetry and other prostate cancer related p arameters on apical positive surgical margins after radical prostatectomy..Method: A retrospective study was performed on 82 patients from 2012 to 2017 who were diagnosed as prostate cancer with no metastasis by prostate biopsy in o ur hospital.All these patients were eligible for surgical indications.All pelvic para meters including anteroposterior pelvic brim distance,interspinous distance,apical pr ostate depth,symphysis angle were measured on preoperative MRI.Combining other parameters(Ages,BMI,PSA,Prostate volume,Surgical Gleason score,Pathological stag e,Opration method),univariable analysis were used to investigate the relationship be tween the parameters and apical positive surgical margins.Multivariate logistic regr ession was used to assess independent risk factors for apical positive surgical mar gins.Result: :24 cases had apical positive surgical margins.Result from univariable anal yse shows that there were no significant difference in ages,BMI,PSA,prostate volu me,anteroposterior pelvic brim distance,interspinous distance and symphysis angle between patients with and without apical positive surgical margins.Multivariate logi stic regression shows that apical prostate depth,surgical Gleason score,pathological stage and opration methods were independent risk factors for apical positive surgi cal margins.Patients with apical prostate depth greater than 28 mm,surgical Gleaso n score more than 7,pathological stage higher than T2 and choosing open opration s have higher rates of apical positive surgical magins.Conclusion:Apical prostate depth greater than 28 mm,Gleason score more than 7,Pa thological stage higher than T2,Choosing open oprations are independent risk facto rs for apical positive surgical margins.
