P-wave Duration And P-wave Dispersion Predict Recurrence In Persistent Atrial Fibrillation Patients After Radiofrequency Ablation

Objective: To analysis the relationship between P-wave duration well P-wave dispersion of persistent atrial fibrillation patients after radiofrequency catheter ablation in early and late recurrenceMethod:We selected 80 persistent atrial fibrillation patients for the first time of radiofrequency ablation(46 men,34 women,average age 60.6±8.1).We followed them for 9.3±2.9 months and divided them into two groups to contrast:early recurrence and early unrecurrence,late recurrence and late unrecurrence.We defined the recurrence in three months for early recurrence,and after three months for late recurrence.All patients would be recorded 12 lead electrocardiogram in 24 hours after radiofrequency ablation.We measured the P-wave duration of 12 lead electrocardiogram and calculated the maximum P-wave duration(Pmax),the minimum P-wave duration(Pmin),and the deviation calculation between Pmax and Pmin,that was P-wave dispersion(Pd).Through regular follow-up,we obtained the rate of early or late recurrence.Results: There were 17 patients who appeared early recurrence,and compared with early unrecurrence,they had significantly larger Pd(74.35±17.78vs60.73±18.37 P=0.008).During the follow-up period,they were 12 patients with delayed recurrence(70.6%)in the early recurrence patients,and We did not find that early recurrence was a predictor of late recurrence.They were 14 patients with later recurrence,and they had significantly larger Pd compared with late unrecurrence(75.71±16.49vs61.06±18.59 P=0.008).The area under the ROC curve of Pd was 0.728,by observing the different threshold,Pd≥60ms displayed a certain value in predicting late recurrence of persistent atrial fibrillation,it had 85% sensitivity,50%specificity,positive predictive value of 26.7%,and negative predictive value of 94.3%.Conclusions:Pd was associated with early and late recurrence of persistent atrial fibrillation,Pd ≥60ms had a certain value in predicting late recurrence of persistent atrial fibrillation after radiofrequency catheter ablation,and the negative predictive value was higher.
