Objective:To investigate the efficacy and safety of 50 mg and 75 mg clopidogrel in the treatment of non-cardiac acute cerebral infarction for anti-platelet effects,and to guide clinical individualized medication.Metheds:A total of 60 inpatients in our Department of Neurology were selected.All were confirmed by CT or MRI as primary acute cerebral infarction.NIHSS scores ranged from 4 to 8 and had not previously been administered clopidogrel.They were randomly divided into low-dose group and high-dose group,both 30 patients,and were given clopidogrel 50 mg and 75 mg respectively.The NIHSS scores,platelet aggregation rates,residual platelet function(MAADP),side effects of the drug,and clinical bleeding events before treatment?day 1?and 14th day were compared.Results:1.During the treatment period,4 patients in the low-dose group and 3 in the high-dose group dissociated from the study because of the patient's request for automatic discharge or refused to reexamination.No adverse events such as bleeding,liver damage,and reduced platelet count occurred during the treatment period.After 14 days of observation and treatment,there was no recurrence of short-term cerebral infarction in both groups.2.There were no significant differences in NIHSS scores,platelet aggregation rates,and MAADP between the two groups before and after treatment of 14 days?P>0.05?.Both NIHSS scores and platelet aggregation rates were significantly decreased after treatment,the differences were statistically significant?P<0.05?.There was no significant difference in MAADP between the two groups before and after treatment?P>0.05?.There were also no significant differences in the bleeding risk and thrombosis risk between the two groups?P>0.05?.Conclusions:In the pilot study,the preliminary results indicate that there were no significant differences in antiplatelet efficacy and safety for patients with mild acute cerebral infarction using 50 mg and 75 mg clopidogrel and their long-term efficacy needs further study.This study provides therapeutic choices of antiplatelet drugs in certain special groups.
