Purpose To effect the analgesic effect of betamethasone in total knee arthroplasty(TKA).Methods :the study was a prospective, randomized, and double blindtrial and was operated by the same group of surgeons in 60 patients with TKA,according to the methods of analgesia were divided into three groups: Group One(periartieular injection with compound betamethasone),GroupTwo(intramuscular injection with compound betamethasone)and Group Three(the control group). The changes of VSA score,CRP,ESR and complication rate were compared between the three groups. Results: There were no signifcant differences in VAS(24,36,48,72 hours after operation),between Group One and Group Two(P>0.05),but all of them were significantly higher than those Group Three. There were no signifcant differences in ESR and C-reactive protein(CRP)(3day after operation),between Group One and Group Two(P>0.05),but all of them were significantly higher than those Group Three.nausea, vomiting, irritability, wound healing, wound infection rate were not statistically increase with the use of compound betamethasone. Conclusion: intraoperative local and intramuscular injection of compound betamethasone can effectively relieve pain after total knee arthroplasty, and reduce the stress response to surgical trauma, facilitate early postoperative rehabilitation exercise.
