Objective:To investigate whether there is a correlation between the shear wave velocity(SWV)of the median nerve and the nerve conduction and the nerve cross-sectional area(CSA)at the carpal tunnel inlet in the diagnosis of carpal tunnel syndrome(CTS).Methods :Sixty two wrists of 40 patients with CTS were examined by two-dimensional ultrasound and and real-time shear wave elastrography(SWE).The CSA and median nerve cross-sectional SWV at the carpal tunnel inlet were measured and recorded.Observe the ultrasound echo and structural changes in the median nerve of the longitudinal section,measure and record the longitudinal SWV.Electromyography(EMG)examinations and nerve conduction measurements were performed.Distal movement latency(DML)of the Wrist Abductor pollicis breviswas was recorded.All measurement data were statistically processed using SPSS 21.0 statistical software.When P<0.05,the correlation was considered statistically significant.Result:1.There is a positive correlation between the longitudinal SWV of the median nerve and DML(r=0.359,P <0.01).There is a positive correlation between the cross-sectional SWV of the median nerve and DML(r=0.329,P<0.01).2.There is a positive correlation between the longitudinal SWV of the median nerve and the median nerve CSA(r=0.488,P <0.01).There is a positive correlation between the cross-sectional SWV of the median nerve and the median nerve CSA(r=0.456,P <0.01).3.There is a positive correlation between DML and the median nerveCSA(r=0.253,P < 0.05).Conclusion:Neural cross-sectional area(CSA)and shear wave velocity(SWV)can show the structural changes and Stiffness changes of the median nerve in carpal tunnel syndrome(CTS),and the correlation with changes in nerve conduction measurements can indicate abnormalities of nerve conduction function.Therefore,it is of great significance to explain the evolution of the complex pathophysiology of disease and guide clinical treatment.
