ObjectiveBy comparing the clinical data of da Vinci robotic radical hysterectomy and laparoscopic radical hysterectomy,to explore the feasibility, safety and the advantages of da Vinci robotic radical hysterectomy.MethodsA retrospective analysis was performed on clinical data of 108 patients who were diagnosed with FIGO stage ?B1~?A2 cervical cancer and underwent radical hysterectomy performed by the same surgery team during February 2016 to February 2017 in Department of Gynecology of the First Affiliated Hospital of Chongqing Medical University.50 cases da Vinci robotic radical hysterectomy(RRH group)and 58 cases laparoscopic radical hysterectomy(LRH group)were compared among baseline characteristics, operative time, intraoperative blood loss, blood transfusion rate, postoperative anal exhaust time, indwelling drainage tube time, indwelling catheter time, postoperative hospital day, postoperative complications and the short-term outcomes.ResultsAll of the procedures were completed successfully without any conversion to laparotomy. There were no great vessels injuries or injury of bladder, ureter, intestines and other adjacent organs.Compared with the LRH group, the RRH group showed significantly shorter operative time, reduced blood loss, lower blood transfusion rate, shorter time of indwelling drainage tube and shorter time of indwelling catheter[(145.2±42.7)min vs(160.7 ± 28.5)min,(93.8±42.6)ml vs(127.2±100.1)ml,0 vs 12.1%,(4.1±1.5)d vs(4.6±1.2)d,(13.9±5.5)d vs(16.6±6.6)d](P <0.05).The number of dissected lymph node, postoperative anal exhaust time, postoperative hospital day, postoperative complications and short-term outcomes were similar in the two groups(P >0.05).Conclusions1.It is safe and feasible for the da Vinci robotic radical hysterectomy.2.Da Vinci robotic radical hysterectomy has the advantages of shorter operation duration, less bleeding, lower blood transfusion rate, shorter time of indwelling drainage tube and shorter time of indwelling catheter when compared with laparoscopic radical hysterectomy.3.There was no significant difference in the incidence of postoperative complications of cervical cancer treated by da Vinci robot and laparoscopy.4.The short-term outcomes of robotic surgery for FIGO stage ?B1~?A2 cervical cancer is comparable to that of laparoscopy, and the long-term outcomes should be further followed up.
