Clinical Analysis Of Serum Alpha Fetoprotein And NLR In 165 Patients With Acute-on-chronic Hepatic Failure

Objective Liver self-healing and degree of the inflammatory response in the prognosis of acute-on-chronic liver failure outcome is of great significance.However,as regards on APF and NLR improvement and deterioration groups in ACLF group specific function is not fully clear.Therefore,this paper mainly evaluates and verifies the specific prognostic value of serum alpha-fetoprotein and neutrophils and lymphocyte ratio in patients with ACLF..Methods Use of the second affiliated hospital of chongqing medical university search system,electronic medical records department,digestive department and other departments in our hospital infection between 2015.1 to 2016.12 months up to the standard of study,165 patients with ACLF retrospective cross-sectional study,divided into relief and deteriorating group,two groups of patients were given antiviral,comprehensive protect liver therapy treatment,collect patients admitted to hospital 24 hours of serum AFP and NLR numerical test,all patients were clinic or telephone follow-up to 12 weeks of admission;SPSS22 software was used to analyze the data.Results There was a statistical difference in AFP and NLR between the two groups.The difference was statistically significant(P<0.01).The AFP of serum at the Area Under Curve of Receiver Operating Characteristic was 0.77;The area under the NLR curve is 0.26,and the truncation value of AFP is higher than that of NLR..Also need to be noticed in the single factor index,alleviate group and deteriorating Cr within 24 hours after admission,TBIL,INR,PTA statistical differences,and MELD score,after 12 weeks of observation period of upper gastrointestinal bleeding,hepatic encephalopathy,hepatorenal syndrome,ascites and spontaneous peritonitis complications on statistical differences.Conclusion There were many factors influencing the prognosis of ACLF,among which,there were significant differences in the prediction of ACLF disease in the single factor and in the AFP and NLR indicators.AFP is more accurate than NLR,and AFP can be used as a prognostic indicator.However,more randomized,large-sample prospective trials are still needed to further validate the relationship between other indicators.
