Objective:In recent years,the incidence of hyperuricemia(HUA)in China has been increasing significantly,which may be related to the continuous development of society,the increase of personal unhealthy living habits and changes in diet structure.At present,a large number of studies have found that hyperuricemia has a close relationship with atherosclerosis.But the medical community still needs to explore whether there is a clear relationship between hyperuricemia and atherosclerosis.The purpose of this study was to investigate whether there was a correlation between the level of serum uric acid(SUA)in the blood of middle-aged and elderly people and carotid atherosclerosis.Methods:A total of 334 people aged 40-75 years who underwent physical examination in the Physical Examination Center of The Affiliated Hospital of Qingdao University from January 2016 to February 2017 were selected,including 191 males and 143 females.All the selected medical examiner were collected general data,including age,sex,smoking history,drinking history,chronic disease history,family history,and other medical history and were measured SUA level,fasting plasma glucose(FPG),triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)in our clinical laboratory by drawing 4ml fasting venous blood and testing.High-resolution ultrasonography was used to record the thickness of the carotid intima-media thickness(CIMT),presence of plaque,and plaque size.All the selected medical examiner were divided into two groups based on the SUA level : hyperuricemia group and non-hyperuricemia group.The above indices were compared between the two groups.The association between carotid atherosclerosis and SUA level was analyzed in all subjects.Results :(1)The hyperuricemia group had lower age and FPG levels than the nonhyperuricemia group(t =4.586,2.019,P<0.05).The proportion of males was higher than the non-hyperuricemia group(?2 =30.344,P<0.05).The levels of LDL-C,TC,TG and CIMT were significantly higher than those of the non-hyperuricemia group(t =2.174~4.297,P <0.05).There was no significant difference in HDL-C levels between the two groups(P >0.05).(2)The SUA levels of all subjects were positively correlated with TG,LDL,TC,and FPG(r =0.175-0.426,P <0.05),and negatively correlated with age(r =-0.226,P <0.05)and had no correlation with HDL-C(r =-0.057,P >0.05).(3)Multiple linear regression analysis of CIMT impact factors showed that age,SUA,TG,and FPG had an impact on CIMT.With age,SUA,TG,and FPG increased,CIMT also gradually increased.(4)Univariate logistic regression analysis of risk factors for carotidatherosclerotic plaques showed that when SUA was considered as a continuous variable,the probability of plaque formation increased with increasing SUA levels(OR = 1.003,P< 0.05);When SUA were grouped in quartiles,the difference was statistically significant between the 350.1-429.1 ?mol/L group,the >429.1 ?mol/L group and the <277.0 ?mol/L group.(OR = 2.202,2.120,P<0.05).The multivariate logistic regression analysis showed that the presence of carotid atherosclerotic plaque was associated with high SUA level(OR =1.003,95% CI =1.001-1.006,P<0.01).Conclusion:Hyperuricemia is a high-risk pathogenic factor for carotid atherosclerosis in middle-aged and elderly people.
