| ObjectTo explore the feasibility of international standardized nursing language-Omaha problem classification system used in the patients with temporary enterostomy;to develop a nursing problem assessment system for the patients with temporary enterostomy in the framework of Omaha classification scheme,and provide a comprehensive and scientific assessment tool for clinical nurses and community nurses;to find out the existing or potential problems in nursing care,so as to select the correct intervention types and intervention direction,the implementation of targeted nursing measures and provide program working modes for clinical and community nurses;this assessment system for empirical research,to investigate the reliability and validity of the assessment system.Method60 nursing records of patients with temporary enterostomy from March to December2016 ingastrointestinal surgery and emergency general surgery,affiliated hospital of Qingdao University,were selected by convenient sampling method.The language of nursing problems of patients with temporary enterostomy described in nursing records was extracted by content analysis method,and then the degree of consistency with each item of Omaha classification scheme was discussed by cross-control method.On the basis of literature analysis,through Delphi method and combining with the unique or prominent characteristics of patients with temporary enterostomy,a nursing problem assessment for the patients with temporary enterostomy in the framework of Omaha classification scheme is constructed.To investigate the 30 cases of patients with temporary enterostomy from June to December in stoma specialist outpatient from June to December of 2017 and measured the reliability and validity of the assessment system.ResultsThe language used to describe the nursing problems of patients with temporary enterostomy in nursing records was compared with the various entries in the Omaha system.After two rounds of Delphi,the nursing assessment system of temporary enterostomy patients was constructed,which included 4 first level indicators,29 secondaryindicators and several tertiary indicators.The first round of positive coefficient is 100%,the second is 84.2%,expert authority is 0.828,and important assignment mean>3.5.The variation coefficient was <0.25,and the concordance coefficient of the three indexeswere 0.371 and 0.262 respectively,carry on test of significance and statistically significant.The coefficient of Cronbach’s coefficient was 0.841,I-CVI was 0.83 ~ 1,S-CVI was 0.91.ConclusionsThis study confirmed that the Omaha system can be used as a standardized language for the care of patients with temporary enterostomy.After the content of the Omaha classification scheme is debugged according to the characteristics of this group,the assessment system is clear in structure and comprehensive in terms of science.To provide a reliable assessment tool for clinical and community nurses to assess the existing or potential nursing problems of patients with temporary enterostomy,so as to select the correct type and direction of intervention according to the intervention system.Provides the procedure work pattern for the clinical or community nursing staff. |
