| Objective: Evaluation of High Resolution Melting(HRM)for the sensitivity and specificity of Mycobacterium tuberculosis to detect four kinds of anti-tuberculosis drug resistance.Using the Xpert-MTB/RIF method as control,to compare HRM with Xpert-MTB/RIF in the diagnostic efficacy of Mycobacterium tuberculosis for the detection of rifampin resistance.Methods: The procedures of acid-fast stain smear,bacterial culture,identification of bacterial types,drug susceptibilities,HRM and Xpert-MTB/RIF were performed on clinical specimens from 300 patients with tuberculosis.The results of HRM were compared with the standard DST and then analyzed.The evaluation of High Resolution Melting(HRM)for the sensitivity and specificity of Mycobacterium tuberculosis detected four kinds of anti-tuberculosis drug resistance.Using the Xpert-MTB/RIF method as control,HRM was compared with Xpert-MTB/RIF in the diagnostic efficacy of Mycobacterium tuberculosis to detect rifampin resistance.Results: In this study,a total of 300 cases of positive smear were collected.A comparison was made between HRM and Xpert MTB/RIF.The consistency of the two methods was 85.0%.Compared with the traditional drug sensitivity test(DST),the sensitivity and the specificity of HRM rifampicin kit were 100% and 79.6%,respectively.The sensitivity and the specificity of HRM isoniazid kit were 60.7% and 85.8%,respectively.The sensitivity and the specificity of HRM quinolone kit were 75% and 94.9%,respectively.The specificity of HRM second-line injectable drug kit was 99%.The specificity of the four detection kits of the HRM are high,but the sensitivity is not.Conclusion: In this study,HRM has relatively high sensitivity and specificity in the detection of Mycobacterium tuberculosis resistance to isoniazid,rifampicin,fluoroquinolones,and second-line injection antituberculosis drugs.The consistency between HRM and Xpert MTB/RIF for rifampicin resistance testing is relatively high.But DST as the gold standard,HRM has high sensitivity and low specificity. |
