Objective: To investigate the effect of single and double stent assisted coil embolization in the treatment of vertebral artery dissection.METHODS: Retrospective analysis of vertebral artery dissection patients treated with stent-assisted coil embolization in our hospital and combined external hospital from January 2013 to May 2017 was performed.60 patients were divided into double-stent group and single-stent group. There were 34 cases in the stent group and 26 cases in the single stent group). The observation group received double stent assisted coil embolization, and the control group received single stent assisted coil embolization.Observe the treatment of the two groups, immediate postoperative, aneurysm embolization and aneurysm recurrence at follow-up.Results: The operations of the two groups were successfully completed. All patients had no complications such as stent dislocation and aneurysm rupture. The immediate embolization rate of aneurysm was 82.35%(28/34)in double stent group, which was higher than 53.85% in single stent group.(14/26),the difference was statistically significant(P<0.05);postoperative imaging was followed up for 3-6 months, showing that the complete embolization rate was not statistically different between the two groups(P>0.05);Clinical follow-up of 6-8 months showed that the proportion of recurrences in the double-stent group was 5.88%(2/34)lower than that in the control group 30.77%(8/26),with statistical significance(P<0.05).The proportion of recurrences in patients with double stent combined with posterior inferior cerebellar artery involvement was 14.29%(1/7)lower than that in single stent group 66.7%(4/6),with no statistical difference;all patients had aneurysm with PICA Thirteen patients were involved, among which the relapse rate was 38.46%(5/13),which was 10.37%(5/47)higher than the recurrence rate without PICA involvement. There was a statistically significant difference(P<0.05).Conclusion: Double stent-assisted coil embolization for vertebral artery dissecting aneurysms can improve the aneurysm complete embolization rate and reduce the postoperative recurrence rate. The recurrence rate of dissecting aneurysms involving PICA is higher. It is inferred that aneurysm combined with posterior inferior cerebellar artery Involvement is a risk factor for recurrence.
