Objective:To explore the safety of lower oxygen administration during the maintenance of general anesthesia in children.Method:The study was approved by the hospital ethics committee, informed the guardian of children and signed the informed consent for 75children accepted selective laparoscopic surgery in general anesthesia. They were scheduled and divided into 3 groups used a random number table:Group C40?FiO2=40%?,Group C30?FiO2=30%?and Group C22?FiO2=22%?.The time before anesthesia induction was T1,the time when stoped the peritoneoscope was T2,recorded the SpO2,HR and rSO2 between T1 and T2.Extracted the radial artery blood for arterial blood gas analysis atT2,recorded the PaO2,PaCO2,and then calculated the OI,PA-aO2,RI and Pa O2/PAO2,and recorded the time of extubation and the stay time in the PACU and the postoperative nausea and vomiting, and surgical site infection. When SpO2<94%or rSO2 decreased by20%,or intraoperative blood loss exceeded by 20%of blood volume, excluded the case. Results:During the maintenance, there were 2 cases excluded in Group C22,and no case was excluded in either group C400 or C30,there was no significant difference?P>0.05?.There was significant difference in PaO2,PAO2,PA-aO2,RI,PaO2/PAO2 at T2?P<0.05?,there was no significant difference in PaCO2 and OI at T2?P>0.05?.There was no significant difference in time of extubation and the stay time in the PACU and the postoperative nausea and vomiting and surgical site infection?P>0.05?.Conclusion:On the premise of clinically monitoring and appropriate ventilation model, that the children with normal lung function inhale lower concentration of oxygen is safety, the oxygen concentration can be set from22%to 40%.With the increase of oxygen concentration, the lung gas exchange function may be injured,in a dose-dependent manner. The lower oxygen concentration will not increase postoperative nausea and vomiting and surgical site infection.
