Object: In this study,we investigates the risk factors that affect the survival and prognosis of patients after radical cystectomy, and provides a theoretical basis for postoperative recovery of patients with muscular invasive bladder cancer. Methods:The clinical data and follow-up data of patients who underwent radical cystectomy at the Second Hospital of Hebei Medical University from September 2012 to December 2017 were retrospectively collected and analyzed. The factors analyzed included patient age, gender, chronic disease, surgical procedure, operation time,TNM staging, lymph node metastasis, invasion of ureter, urethral stump Tumor size, tumor multifocal and postoperative neoadjuvant therapy. Results: Follow-up of 68 patients,of which 7 patients with incomplete information.Follow-up data of 61 patients with complete. The follow-up period ranged from 3 to 60 months.Univariate analysis by Log-rank test showed: T staging, tumor size, urinary diversion, ureter invasion, urethral residuals.Whether the end involvement, lymph node metastasis, postoperative neoadjuvant chemotherapy, these factors P<0.05,the difference was statistically significant. The results of Cox multivariate analysis showed that there were significant differences in the prognosis of patients between the mode of urinary diversion,T staging, lymph node metastasis and postoperative neoadjuvant chemotherapy(P <0.05).Conclusions:Multivariate analysis of the Cox proportional hazards model combined with Log-rank test of univariate analysis showed that T staging, lymph node metastasis, and postoperative neoadjuvant chemotherapy were factorsinfluencing patients' prognosis. The main factors are selected from many factors, and then effective interventions are taken to reduce patient mortality and improve survival.
