Objective:For epithelial ovarian cancer(EOC),postoperative recurrence is a significant factor to poor prognosis.The study is aim to investigate what makes tumor resistance to platinum-based chemotherapy after initial successful therapy,then to provide the basis for clinical diagnosis and treatment.Method: The data of 170 patients with recurrent epithelial ovarian cancer between May 2010 to October 2017 was retrospectively analyzed,whose primary treatments were completed in Fourth Hospital of Hebei Medical University as well.A chi-square test was used counting materials.Wilcoxon Rank Sum Test was used in measurement datas.The risk factors were determined by multivariate logistic regression analysis.Result:(1)The median recurrent interval of 170 patients with recurrent epithelial ovarian cancer was 13 months,35 cases of whom were platinum resistance recurrence,accounting for 20%.(2)Factors associated with platinum resistant relaps by univariate analysis were as follows:ascitic volume?1000ml(P=0.045),advanced ovarian cancer(?-?)(P=0.045),NACTIDS(P=0.025)and the increasing chemotherapy courses(P=0.044);(3)Logistic regression analysis showed that ascitic volume>1000ml(P=0.045,OR=2.331,95%CI=1.019-5.334),NACT-IDS(P=0.026,OR=2.608,95%CI=1.119-6.007)were the independent risk factors associated with platinum resistant recurrence.Conclusion:(1)The recurrence rate of platinum resistance after primary treatment was 20%.(2)Platinum resistance recurrence may be associated with the stage of ovarian cancer and chemotherapy courses.(3)Preoperative ascitic volume?1000ml and NACT were independent risk factors affecting the platinum resistance recurrence of epithelial ovarian Cancer.(4)Early diagnosis,selection of appropriate treatments and standard postoperative chemotherapy may slow down the recurrence of platinum resistance.
