PART1 THE VALUE OF DIFFERENT APPARENT DIFFUSION COEFFICIENTS IN THE EVALUATION OF BENIGN AND MALIGNANT ENDOMETRIAL LESIONSPurposeTo evaluate the different values of apparent diffusion coefficient of DWI in differential diagnosis of uterine endometrial lesions.To find the optimal ADC values of distinguishing benign endometrial lesions from malignant endometrial lesions.MethodsSixty-three malignant endometrial lesions and 15 benign endometrial lesions which performed with 1.5 T MR1 were retrospective analysis.The mean apparent diffusion coefficient(mADC),the relative apparent diffusion coefficient(rADC)(Internal obturator muscle as the reference site)and the minimum apparent diffusion coefficients(minADC)were measured.And all the ADC values between different endometrial lesions were compared.Then statistical analysis was performed.ResultsThe mADC,rADC and minADC values of benign and malignant endometrial lesions were(1.47±0.25)×10-3 mm2/s,0.97±0.15,(1.18±0.26)×10-3mm2/s and(1.02±0.18)×10-3mm2/s,0.69±0.3,(0.75±0.18)×10-3mm2/s.All of the ADC values of malignant endometrial lesions were significantly lower than those benign lesions.Based on ROC curve,the AUC was 0.94,0.92,and 0.91 respectively.The maximum YI was 0.72,0.74,and 0.71 respectively.And rADC=0.85 can be used as the best threshold for identifying benign and malignant endometrial lesions.ConclusionThe mADC,rADC and minADC measurement had a potential ability to differentiate between benign and malignant lesions of the endometrium.Using rADC values to distinguish endometrial benign lesions from malignant lesions was more accurate.PART2 THE VALUE OF DIFFERENT APPARENT DIFFUSION COEFFICIENTS IN THE EVALUATION OF ENDOMETRIAL ADENOCARCINOMA WITH DIFFERENT HISTOLOGICAL GRADEPurposeTo evaluate the different values of apparent diffusion coefficient of DWI in diagnosis of different histological grade of endometrial adenocarcinoma.MethodsFifty-three endometrial adenocarcinoma(6 well differentiated tumors,34 moderately differentiated tumors,and 13 poorly differentiated tumors)which performed with 1.5 T MRI were retrospective analysis.The mean apparent diffusion coefficient(mADC),the relative apparent diffusion coefficient(rADC)(Internal obturator muscle as the reference site)and the minimum apparent diffusion coefficients(minADC)were measured.And all the ADC values between different histological grade were compared.Then statistical analysis was performed.ResultsThe mADC and rADC could significant distinguish well differentiated(G1),moderately differentiated(G2),and poorly differentiated(G3)endometrial adenocarcinoma.But minADC could only differentiate G1 from G2 or G1 from G3.ConclusionThe mADC and rADC values could help to judge all the histological grade of endometrial adenocarcinoma.
