Efficacy And Prognostic Factors Of Three Dimensional Conformal Radiation Therapy With Simultaneous Integrated Boost (CRT+SIB) And Whole Brain Irradiation Plus Three Dimensional Conformal Radiation Therapy With Boost Relied On Sequential Treatment Design (

Objective:The aim of this study is compare the efficacy of three dimensional conformal radiation therapy with simultaneous integrated boost?CRT+SIB?and whole brain irradiation plus three dimensional conformal radiation therapy with boost relied on sequential treatment design(CRT_SEQ)in treatment of brain metastases from non small cell lung cancer?NSCLC?,and prognostic factors were analysised.Methods:From June 2012 to June 2014,152 patients with brain metastases from NSCLC in our hospitalwere selected and randomly assigned?1:1?toCRT+SIB?n=76?and CRT_SEQ?n=76?,the clinical efficacy was compared.Overall survival were calculated and outcome prognostic factors were evaluated.Results:The local complete remission rate of the CRT+SIB group was 72.4%?55/76?,the local complete remission rate of CRT_SEQEQ group was 44.7%?34/76?,the difference was statistically significant?P<0.05?;The 1 year survival rate,2 year survival rate and 3 year survival rate of the CRT+SIB group were all higher than those of the CRT_SEQEQ group?P<0.05?;Multiple factor analysis:tumor size,primary lesion and metastasis,and radiotherapy were prognostic factors?P<0.05?.Conclusion:three dimensional conformal radiation therapy with simultaneous integrated boost in treatment of non small cell lung cancer brain metastases has better clinical efficacy,it is an independent factor affecting prognosis,tumor size,primary lesion and metastasis were also prognostic factors.
