| Objective:To investigate the correlation between the indexes of 24 hour ambulatory blood pressure monitoring and the recurrence of cerebral infarction.Methods:The 148 cases who sufferring from the primary hypertension accompanied with acute cerebral infarction were collected from October 2016 to October 2017 in the first affiliated Hospital of Shihezi University medical School.According to whether the occurrence of cerebral infarction is the first time or not,77 and 71cases were divided into first onset cerebral infarction group(primary group)and recurrent cerebral infarction group(relapse group),respectively.1.Analyze the difference of ABP,BPV,BPCR,BPLV and MBPS between primary group and relapse group.2.Logistic regression analysis to analyze the effects of each index on the recurrence of cerebral infarction.3.According to MABP,primary group and relapse group were divided into normal blood pressure group,mild hypertension group and severe hypertension group respectively,and then analyzed the differences of 24 h SBP,24 h SBPV,MRPS and NHISS between groups.4.The interaction interaction model of epidemiology was applied to analyze the interaction between 24 h SBP and MRPS on recurrence of cerebral infarction.Results:1.There was no significant difference in relevance ratio of recurrence of cerebral infarction between primary group and relapse group,but there was significant difference in relevance ratio of recurrence of cerebral infarction between primary group and relapse group in elderly people.2.The average blood pressure of 24 h SBP,day SBP,24 h DBP and day DBP in relapse group were higher than primary group(P<0.05).3.The 24 h SBPV,d SBPV,24 h SBPLV,d SBPLV in relapse group were higher than primary group(P<0.05),and The systolic pressure and diastolic pressure of BPCR in relapse group were lower than primary group(P<0.05).4.The logistic regression analysis showed that 24 h SBP,24 h SBPV,d DBP,and MBPS were the risk factors of recurrence of cerebral infarction.5.After divided into gropus according to the MABP,the results suggested that MBPS of the relapse group was higher than the primary group(P < 0.05)at the same or slightly lower blood pressure level(P < 0.05),and the NHISS score of normal blood pressure group which belonged to the relapse group was higher than the primary group(P <0.05).6.Epidemiological case control study interaction model showed that morning peak blood pressure and 24 h SBP interacted on each other in the recurrence of cerebral infarction.Conclusion:1.The age in excess of 65,high mean blood pressure,high blood pressure variability,high blood pressure load values,disappearance of circadian rhythm and high morning peak blood pressure are the risk factors of RCI.2.MBPS was the independent risk factors of RCI.3.24 h SBP and MBPS were interacted on each other in the RCI. |
