OBJECTIVE: The needle pricking has a unique effect on the treatment of male infertility,but the mechanism is not yet clear.In this study,the relations among oxidative stress,abnormal sperm energy metabolism and mitochondrial mediated sperm apoptosis are analyzed by cytological and molecular biological detection.Besides,we also researched the mechanism of needle pricking on the treatment of male asthenozoospermia in reactive oxygen species,mitochondrial function,sperm apoptosis,etc.METHODS:Single-blind randomized controlled trial design.From June 2016 to January 2018,54 cases male patients with asthenozoospermia were randomly divided into three groups,each of 18 cases.Cases in drug treatment group received antioxidant therapy with coenzyme Q10 120mg/day,vitamin E 40mg/day and vitamin C 80 mg/day.Cases in needle pricking group were received the needle pricking one time a week,we take the sacral plexus nerve stimulation point as the main point.Cases in combined treatment group received coenzyme Q10 120mg/day,vitamin E 40mg/day,vitamin C 80mg/day and the needle pricking one time a week.Each courses of treatment was 3months.RESULTS:(1)After 3 months,each group of sperm density,sperm count,sperm forward progression percentage,sperm ROS,sperm normal MMP percentage,late apoptosis rate and apoptosis rate of sperm improved obviously(p<0.05).(2)After 3months,sperm density,sperm forward progression percentage and sperm normal MMP percentage in the needle pricking group were significantly better than that in the drug group(p<0.05).(3)The sperm density,sperm count,sperm total motility,sperm forward progression percentage,sperm ROS,sperm normal MMP percentage,loss of sperm MMP percentage and early apoptosis rate of sperm in the combined treatment group were significantly better than that in the drug group(p<0.05).On the other hand,it was significantly better than the needle pricking group in sperm total motility,sperm forward progression percentage and early apoptosis rate of sperm(p<0.05).CONCLUSION: By the needle pricking,it can adjust the function of nervous system,then improves the ability to regulate a stress response and reduces the productionof ROS,so as to protect the integrity of sperm DNA and maintain normal mitochondrial membrane potential,which in order to reduces the apoptosis rate of sperm and enhance the motility and density of sperm.More importantly,the combination therapy----needle pricking combines with vitamin C,vitamin E and coenzyme Q10 treatment significantly decrease the sperm ROS,improved the function of the testicular or epididymal and change the sperm microenvironment.Besides,it significantly reduces oxidative stress damage,protects sperm DNA and mitochondrial function in order to improve sperm motility and density.
