Benefit Analysis Of En Bloc Transurethral Resection With Laser In The Treatment Of Non-muscle-invasive Bladder Tumor

Objective:To investigate the efficacy and safety of transurethral laser en bloc resection of bladder tumor and standard transurethal resection of bladder tumor(TURBT)in patients with non-muscle-invasive bladder tumor.Methods:A total of 60 patients having surgical treatment in our hospital from September of2014 to December of 2016,according to the surgical methods,were divided into Laser group(n=30,transurethral laser en bloc resection of bladder tumor)and TURBT group(n=30,TURBT).The operation was performed by single seasoned surgeon(Wang Dongwen)with standard procedure.The preoperative ? intraoperative and postoperative characteristics were compared between two groups.Results:The operations were performed successfully.The experimental group's operaiton time was(78.70±25.81)min;another group's time was(63.37±39.51)min.Laser en bloc resection group was superior to the TURBT group in terms of blood loss [(13.97±6.21)ml vs.(63.13±13.85)ml,P<0.001]?obturator nerve reflex [0.00(0/30)vs.23.33%(7/30),P=0.016]?catheter indwelling time[(3.40±0.89)d vs.(6.13±1.66)d,P<0.001]?bladder irrigation time[(1.80±0.66)d vs.(3.77±1.10)d,P<0.001]?hospital time[(6.10±1.49)d vs.(8.33±2.11)d,P<0.001]?bladder irritation symptom [33.33(10/30)vs.66.67%(20/30),P=0.010] and 1-year recurrence rate [3.33(1/30)vs.26.67%(8/30)P=0.030].There is nodifference between laser group and TURBT group in bladder perforation[0.00(0/30)vs.3.33%(1/30),P>0.05].Conclusion:The results of our study have shown that the laser en bloc resection is a safe,effective and feasible alternative for the treatment of primary non-muscle-invasive bladder carcinoma;moreover,it can provide whole specimen for the pathologic diagnosis.It is worthy of clinical application.
