| ObjectiveStudy the law of syndromes of HBV-acute on chronic liver failure and the influences in traditional Chinese medicine syndromes distribution of sex,age,course of disease and laboratory indexes,andthe effect of the proof on the prognosis of the disease.MethodsPatients diagnosed as HBV-acute on chronic liver failure were enrolled at the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from January 2013 to January 2017,each patient was registered based on standard case and created database in retrospective manner,thus exploring distribution characteristics of TCM syndromes of hepatitis B virus-acute on chronic liver failure,assessing the correlation of TCM syndromes and gender,age,disease course and laboratory indexes,and determining the impact on prognosis of the disease that the TCM syndromes had.Results⑴、HBV-acute on chronic liver failure incidence rate was higher in male than that in female,male to female ratio was 3.56:1;with average onset age(45.07 + 13.11),patients are mainly youth and the proportion was up to 53.33%,and their occupation was most farmers and workers;liver disease duration lasted 10-19 years and even20-29 years,the ratio was 76.19%;the known predisposing factors were generally staying up late and antivira-related factors with the percentage up to 45.71%⑵、105 cases of HBV-acute on chronic liver failure were studied,among which HBeAg positive accounted for 56.19%,while HBV-DNA negative accounted for65.71%,and more complications occurred in these cases,with hypoproteinemia(96.19%),ascites(62.85%),electrolyte disorder(61.90%),hepatic encephalopathy(42.85%),and hepatorenal syndrome(26.66%).⑶、TCM syndromesdistribution mainly presented as the following pattern,with Dampness-heat jaundice 39.04%;Dampness-cold jaundice 26.67%;Plague poison jaundice 2.85%;Qi-deficiency jaundice 2.85%;yin-deficiency jaundice1.91%,Yan-deficiency jaundice 27.62%,Correlation between TCM Syndrome and gender and complications had no statistical significance(P>0.5),while it had significant correlation with ages,disease course and the stage of diseases(P<0.5)⑷、The distribution of TCM syndromes and laboratory indexes: HbeAg,ALT,and AST had no correlation with the distribution of TCM syndromes(p>0.5);HBV-DNA,PTA,TBIL and DBIL had correlation with the distribution of TCM syndromes(p<0.5)⑸、TCM syndromeswere associated with treatment effects(P< 0.05).Outcome of Dampness-heat jaundice was higher than that of other syndrome types,although the mortality rate of Yan-deficiency jaundice was higher than that of other syndrome types.conclusion⑴、Characteristics of population that is susceptible toHBV-acute on chronic liver failure: male is more susceptible than female;patients are mainly youth with their occupation acting as farmers and workers;liver disease duration last long,staying up late and antivira-related factors are most known inducing factors.⑵、HBV-acute on chronic liver failure is often companied with complications and comorbidities.⑶ 、 Gender,complications and comorbidities or laboratory indexes like HBeAg,PT,PTA,ALT and AST had no implications with TCM syndromes⑷ 、 Ages,disease course and the stage of diseases(early/late-stage)or laboratory indexes like HBV-DNA,TBIL and DBIL had association with TCM syndromes to some extent.⑸、Outcome of Dampness-heat jaundice was higher than that of other syndrome types.while mortality rate of Yan-deficiency jaundice was higher than that of other syndrome types,indicating that TCM syndromes had impact on prognosis of thedisease. |
