| Objective:Measurement of left ventricular global function in uremia patients with different dialysis periods by measuring Tei index by tissue doppler ultrasonography.Methods:30 patients with maintenance hemodialysis uremia patients, uremic patients were divided into group A,group B and group C according to different dialysis periods:Group A dialysis time <1 year(n = 10),Group B 1 year ≤ dialysis time≤ 3 years(n = 10),group C dialysis time> 3 years(n = 10),Another 30 healthy volunteers in the same period of our regular physical examination were set as normal control group(group N).The patient group and normal control group in the parasternal left ventricular long axis section under the M-mode ultrasound measured were measured by Simpson’s method under the apical4-chamber view; The transmitral diastolic blood flow peak velocity(VE)and the diastolic blood flow peak velocity(VA)were measured with the pulse doppler mode under the apical4-chamber view, and calculated the E/A ratio; The contraction time(ICT),relaxation time(IRT),ejection time(ET)were measured with the tissue doppler ultrasound mode, using the formula(ICT + IRT)/ ET,calculated the Tei index.Results:1.Conventional two-dimensional ultrasound and M-mode ultrasound measurement parameters comparison:The values of IVST、LVPWT and LVEDD: all the three groups in the patient group were higher than those in the control group, especially in the patient group B and C,the values of the two groups were significantly higher than those in the control group(P<0.05 or P<0.001);The values of LVEF: there were a little differences between the group A with the control group(P>0.05),but there were significant differences between the group B and C with the control group(P<0.05).2.Comparison of parameters of Pulsed Doppler Ultrasound: there was no significant difference in the value of E、A and E/A between the three patient groups and the controlgroup(P>0.05).3.Comparison of tissue Doppler Ultrasound parameters: there was no significant difference in IRT and IRT between patient group A and the control group(P>0.05),ET is shorter than the control group(P<0.05);Comparison of patient group B with the control group and patient group A,the conclusion was prolongation of IRT and shortening of ET(P<0.05);Compared with the control group and the A group, the ICT and IRT were prolonged, and the ET was shortened in the C group(P<0.05);Compared with the control group, the Tei index of the three groups in the patient group was higher, and there have statistically significant differences(P<0.05);Especially in group C,the Tei index increased significantly, and the differences was statistically significant(P<0.001).Conclusion:In patients with uraemia left ventricular systolic function was within the normal range, left ventricular diastolic function was already showing a tendency of decrease, measured by tissue Doppler Tei index can be very good evaluation of uremic dialysis patients with left ventricular total function,To provide more evidence for clinical early intervention. |
