Objective to investigate the dynamic changes and significance of P-selectin(CD62P) in patients with henoch-schonlein purpura(HSP)and henoch-sch?nlein purpura nephritis(HSPN).Methods 1?Detailed recording the clinical data of 80 allergic purpura cases,include in basic information(name?age?gender?treatment and follow-up etc)?clinical manifestation(skin purpura?abdominal pain?joint swelling?hematuria and upper gastrointestinal hemorrhage etc)?laboratory examination(blood and urine routine?blood biochemistry?blood coagulation function?urine microalbumin(U-MALB)?urine alpha1 microglobulin(U?1-MG)?total urinary protein in 24 hours etc).2?After a six months of treatment and follow-up of all the children,we divided them into two groups according to the condition of renal damage,henoch-sch?nlein purpura group(N-HSPN group)and henoch-sch?nlein purpura nephritis group(HSPN group).An other 40cases of healthy children in outpatient medical examination in our hospital we re classified as control group.3?using the flow cytometry technology to test the value of blood and urine CD62P in hospitalization and 8 weeks after treatment.The control group was only detected the blood and urine CD62P on the day of physical examination.4?Compared the blood and urine CD62P in different groups as well as before and af ter treatment.5?Analysis the correlation between the value of urine and blood CD62P.6?Analysis the correlation between the value of urine CD62P and urine alpha1 micro globulin?urine alpha1 microglobulin.7?Compared the clinical manifestations and laboratory results like blood routine?blood coagulation function and immunoglobulin in henoch-sch?nlein purpura group and henoch-schonlein purpura nephritis group.Results 1?Compared with the control group,the value of blood and urine CD62P were significantly increased in N-HSPN group and HSPN group,the difference was statisti cally significant(P<0.05).2?Compared with the N-HSPN group,the value of urine CD62P was obviously increa sed in HSPN group,the difference was statistically significant(P<0.05).However,the blood CD62P in this two groups was not obvious different,there was no statistic significance(P>0.05).3?After 8 weeks of treatment,the value of blood CD62P in the N-HSPN group was decreased than before(P<0.05),and there's no significant difference with the control group(P>0.05);However,the value of blood CD62P in the HSPN group was still higher than the control group(P<0.05),and there's no obvious changes before and after treatment(P>0.05).4?After 8 weeks of treatment,the value of urine CD62P in the N-HSPN group was decreased than before(P<0.05),and there's no significant difference with the control group(P>0.05);the value of urine CD62P in the HSPN group was decreased than before,but it was still higher than the control group,the difference was statistically significant(P<0.05).5?No significant correlation was found between the level of urine and blood CD62P(P>0.05).6?No significant correlation was found between the level of urine CD62P and urine alpha 1 microglobulin?urine alpha 1 microglobulin(P>0.05)7?The children of HSPN group was prone to have an older age,and the ratio of occurrence of persistent severe abdominal pain was higher than the N-HSPN group,both of them had statistical significance(P<0.05).But in the aspects of sex?fecal occult blood test?rash recurrence?platelet count?two D dimer?fibrinogen?immunoglobulin A?complement C3,there was no significant differences(P>0.05).Conclusions 1?The value of blood and urine CD62P in N-HSPN group and HSPN group were higher than the control group,suggesting that CD62P may involved in the pathogenic of henoch-schonlein purpura and henoch-sch?nlein purpura nephritis.2?The value of urine CD62P in the HSPN group were obviously higher than those in the N-HSPN group,but there's no obvious difference of blood CD62P in this two groups,suggesting that blood CD62P and urine CD62P may have a different souces,and urine CD62P can better reflect the renal damage of inhenoch-sch?nlein purpura,and urine CD62P can be used as early mark in predicting the occurrence of HSPN.3?After 8 weeks of treatment,the value of blood and urine CD62P in the N-HSPN group was decline than before,the value of urine CD62P in the HSPN group was relati vely decreased,but it was still higher than the control group,and there was no significant difference of blood CD62P in the HSPN group after 8 weeks,revealed that the duration of inflammation in HSPN group would be longer,the expression level of CD62P was consistent with the changes of diseases variety.and blood?urine CD62P can be used as a noninvasive or minimally invasive index to reflect the condition changes of HSP/HSPN.4?No significant correlation was found between the level of urine and blood CD62P,as well as urine CD62P with urine alphal microglobulin?urine alphal microglobulin.5?The risk factors of kidney damage in henoch-sch?nlein purpura were related to an older age onset?serious and continued abdominal pain.
