The Long-term Curative Effect Follow Up Of Ileal Orthotopic Neo-bladder

Bladder cancer is a kind of very common uropoiesis surgical department of malignant tumor,with the development of science and technology,not only at the request of the curative effect of disease in patients,at the same time has a more urgent desire to improve postoperative quality of life.Then Radical Cystectomy(Radical Cystectomy,RC)and Ileal Neobladder acts as in accord with human body the physiological state of the mode of operation,becoming the first choice for the surgery treatment of invasive bladder cancer.Objective: Through the observation,analysis,to explore Radical Cystectomy and Ileal Neobladder of postoperative patients after treatment effect and complications of follow-up.Methods: Our group are all male patients.Collected 54 patients from January 2008 to January 2014 who accept Radical Cystectomy and Ileal Neobladder,age of 48 to70 years old,average 59 years.Inclusion criteria: preoperative were performed preoperative serum creatinine,blood urea nitrogen,electrolyte urinary tract,urinary tract colour to exceed,vein angiography,CT,cystoscopy,such as inspection,and confirmed by the pathological biopsy of bladder transitional cell carcinoma,postoperative can follow the doctor's guidance,efforts to restore.Postoperative can according to the doctor's advice to our review regularly.Exclusion criteria,exclusion criteria for: no radical excision of bladder surgery patients;All basic conditions have important viscera function is not complete or the whole body is bad,can't tolerate surgery patients;Clotting mechanism disorder.Can't accept long-term follow-up,etc.follow-up: after,6 months,routine check once a year later blood in the urine,blood biochemistry,urinary B to exceed,cystoscope,at the same time check the bladder capacity,residual urine,urine flow rate,measurement data with mean + /-standard deviation(+ /-s),said the count data to positive rate of the number of cases,said,efficient compared with chi-square test,P < 0.05 for the difference is statistically significant.Using(SF-36)scale postoperative life quality were observed.Results : Firstly,with Radical Cystectomy and Ileal Neobladder compared before surgery,surgical treatment group,serum creatinine,blood urea nitrogen,blood electrolyte level indicators are different degree of difference,but the difference is there is no statistical significance(P > 0.05).Secondly the long-term follow-up of urination urine flow mechanics,postoperative bladder capacity of Ileal Neobladder is(470 + /-52)ml,urine flow rate is(17 + /-5.3)ml/s,residual urine is(23 + /-10.4ml,basically satisfy patients with normal urination function.Thirdly,postoperative urinary tract infections,there was a distinct improvement in compared with the preoperative difference was statistically significant(P < 0.05),Forthly,SF-36 scale survey results,the scores are as follows.Role-Physical is 47.24 + /-20.52;Physiological function is 59.28 + /-16.57,Body pain is 61.88 + /-18.04;Vitality is56.17 + /-12.77;58.52 + /-16.83 of General health;57.40 + /-17.79 of Socialfunction;54.47 + /-13.85 to Mental health;Role emotional is 59.31 + /-23.16.Eighty percent of patients who follow-up to the satisfaction of basic are in postoperative existing life quality level.Conclusion: firstly: long-term follow-up to prove Ileal Neobladder in situ technique to improve the kidney function not significant,changes to the electrolyte of no statistical significance,there is no obvious effect.second :postoperative urine flow mechanics and restore fair,to a certain extent to satisfy patients with normal micturition and urinary flow of quality control.Thirdly: on reducing the incidence of urinary tract infection is effective.Fourthly: by SF-36 healthy quality investigation to a degree that patients with satisfactory quality of postoperative life level.So the Ileal Neobladder is a kind of ideal operation for the treatment of bladder cancer.
