Background : With the economic development and social progress,people's living standards improve,the aging of the population continues to increase,the elderly in general due to loose or large areas of tooth loss caused by loss or removal,resulting in missing teeth,affecting life and health.The cases of dentition defect are more common,of which dentinal distal-extension edentulous cases is more difficult to repair,clinical repair methods,including implant repair,removable partial denture repair and fixed-removable combined repair,but removable partial denture repair is still the most important.Dentition free missing,no available abutments at the end,the end of the removable partial denture prone to sinking,accompanied by the end of the abutment torque,the abutment in the loose after the injury,mucosal tenderness,gap at the alveolar ridge easy absorbed.Therefore,the repair of dentinal distal-extension edentulous should focus on reducing the denture on the abutments,mucous membranes and alveolar ridge damage,by designing different clasps on the end of the abutment to reduce the injury,and can delay alveolar ridge absorption.Objective: To evaluation and application of the clinical effects of different clasp retainers on distal-extension edentulous cases when removable partial dentures are used to repair free-end edentulous.Medical records and Methods: The patients with free-end edentulous 11 cases were selected and collected at the Affiliated Stomatological Hospital of Dalian Medical University from January 2016 to January 2017,of which 6(maxillary),7(mandibular),7 cases of bilateral free loss,6 cases of unilateral free loss.The patients also were divided into several groups,including 6 cases of male and female 5 cases,the youngest of 55 years old,the largest 85 years old,with an average age of 68 years old.Asked the patient complained in detail,after clinical examination,to understand the time of patients with missing teeth,with or without the old denture,the old denture,missing teeth and the number of abutments and periodontal conditions,and communicate with patients to understand the treatment intention,removable partial denture of the tooth preparation,preparation of mold,test chewing,and then complete the final denture production,and try wearing denture.Follow-up of patients for 2-10 months,through the clinical examination and the use cases of functional dentition after wearing,to evaluate the effect of design of clasp retainers on terminal abutment tooth for RPD in distal-extension edentulous cases.Results: The 11-patients had good retention of the denture,stable and no obvious tilt,the base of the extension of the edge of the appropriate,soft tissue without tenderness,the median and lateral occlusion is good,no tenderness when chewing.After 2-10 months of denture wearing a follow-up observation,of which 3 cases of mild mucosal tenderness in the chewing,there are 2 cases of functional dyslexia symptoms,they are polished and localized buffer to ease.All 11 patients had no pain or loosening of the abutment.Conclusion: Removable partial denture repair free-end edentulous,according to the end of the abutment of its tooth position,tooth and periodontal condition,the number of missing teeth,appropriate clasp retainers shall be used.This can protect the abutment and mucous membrane,improve the use of effects of denture.
