Objective: To estimate the prevalence of bundle branch block and to explore the risk factors of bundle branch block in elder people of Chinese community.Methods: We selected the target population through cluster random sampling in the Shanghai Gao-hang community between June 2013 and October 2013.Inclusion criteria: Residents who live in the community for more than half a year can actively participate in community medical examination.The elder people are more than 65 years old who have acquired Shanghai Social Security Card.The total random-sampled participants were 3950,including 1745 males and 2205 females,who meet the inclusion criteria.And the average age of all participants were 72 ± 6.58 years old.Professional medical workers record the basic information of participants,including the age,sex,smoking,drinking,and medical history.However the complete blood count,blood glucose,fasting blood glucose,HbA1 C,serum creatinine,urea nitrogen,uric acid,homocysteine,high sensitivity C-reactive protein and other laboratory indicators were tested by biochemistry methods,and test the EF value by using cardiac ultrasound.According to the different types of bundle branch block(No BBB were 3717 cases,LBBB were 53,RBBB were 180).To evaluate the clinical baseline data of different age groups and the ratio of different types of bundle branch block by using the Chi-square test.To explore the risk factors for high incidence of LBBB and RBBB by Logistic regression.In this study,SPSS17.0 was used for statistical analysis,P <0.05 means that the difference between groups was statistically significant.Results:1.The prevalence of bundle branch block was 5.9% in an elder community-dwelling population in the Gao-hang community of Shanghai,among which RBBB was 4.56% and LBBB was 1.34%.The prevalence of BBB was higher in malethan that in female(RBBB 2.56% VS 2.0% P <0.01;LBBB 0.86 % VS 0.481%P <0.01).2-trend test indicated that BBB prevalence is higher with the increasing of age.It always increased significantly with increasing age-related changes.The prevalence of LBBB increased from 0.65% in age 65 years to 2.96%(P <0.01)in very elderly person,however the prevalence of RBBB was from 2.59% to 7.57%(P <0.01).2.In different age groups,2-trend test indicated that The incidence of hypertension,cardiovascular disease(CVD)were significantly higher with the increasing of age.Compared with the elderly,the SBP,fasting blood glucose,HDL-C,Homocysteine,C-reactive protein,serum creatinine,urea nitrogen and uric acid were higher in the very elderly people.This difference between the two groups was statistically significant(P<0.05).3.According to the different types of bundle branch block,we found that the prevalence of hypertension,myocardial infarction,valvular heart disease and serum creatinine in LBBB group was significantly higher than that in control group,and the difference was statistically significant(P <0.05).Compared with the control group,the levels of homocysteine,serum creatinine and uric acid in RBBB group were significantly higher than those in the control group(P <0.05).4.Multiple-logistic regression showed that age,male,hypertension,AMI and valvular heart disease are risk factors of LBBB,and the ORs(95%CI)were1.08(1.04-1.13);2.09(1.11-3.95);1.97(1.01-3.87);5.81(1.62-20.86);5.57(1.15-26.89);0.96(0.937-1.0).However,the age 1.08(1.05-1.11)?male 1.61(1.10-2.35)are risk factors of RBBB.Conclusion:1.The prevalence of BBB prevalence is higher with the increasing of age,and the highest among the very elderly population in Gao-hang community of Shanghai.The prevalence of BBB is more in men than women,and the prevalence of RBBB is more than LBBB.2.There is significant correlation between RBBB and age and males,but not having significant correlation with cardiovascular risk factors.However there was significant correlation between LBBB and age,males,hypertension,myocardial infarction,valvular heart disease and EF.
