| Objective: The purpose of this study is to understand the present situation of psychological contract,competence and turnover intention of ICU Nurses in Three-Grade Class A Hospitals in Nanchang City,and explore the relationship between psychological contract and competence,and turnover intention of the three factors,explore the impact of turnover intention of ICU nurses and provide effective suggestions for hospital management and nursing management of ICU.Reduce the turnover rate.At the same time,through the investigation and analysis of the status of ICU nurses’ competence,this paper provides suggestions and data support for the training of ICU specialist nurses.Method: This study adopts questionnaire survey method,through intensive medicine to sampling in Nanchang city 6 Three-Grade Class A general hospitals(ICU)a total of 221 nurses were investigated by questionnaires,the questionnaire included basic personal information,psychological contract scale of nurse,ICU nurse competency scale and turnover intention scale.The recovered questionnaires were input by Epidata3.1 software,and the statistical results of questionnaires were statistically analyzed by SPSS22.0 and AMOS24.0.The statistical methods used in the analysis were descriptive statistical analysis,single factor ANOVA,LSD and T2 ex post multiple comparison,Pearson correlation analysis,multiple linear regression analysis and AMOS structural equation model.Result:1、The total score of psychological contract was(80.75±20.82),and the scores of hospital responsibility(49.29±15.67),hospital team responsibility score(20.60±7.05),the development of hospital responsibility score(14.15±4.34),the hospital reality responsibility score(14.53±4.90).The nurse responsibility(31.46±8.20),the real responsibility score(6.54±1.95),the nurse team responsibility score(15.34±4.20),the development of nurse responsibility score(9.57±2.63).ICU nurses’ psychological contract scores of different age,professional title,ICU working years,highest education level,overtime work,work preference,monthly average night shift and monthly average income have statistically significant difference(p<0.05).2、The total score of competence in was(168.85±35.12),including professional knowledge score(39.76±8.34),professional skill score(57.86±12.69),professional ability score(55.67±14.03),psychological trait score(15.56±3.41).The scores of ICU nurses with different ages,professional titles,positions,ICU working years,marital status,children,ICU qualification and average monthly income have statistically significant difference(p<0.05).3、The total score of turnover intention was(14.94 ±3.34).The turnover intention Ⅰwas(4.64±1.47),the turnover intention Ⅱ was(4.53±1.49),and the turnover intention Ⅲ was(5.77±1.14).There were statistically significant differences in the score of turnover intention in two aspects: regular overtime or not(p<0.05).4、The correlation coefficient between the psychological contract of ICU nurses and the competence of ICU nurses was-0.248(p< 0.01).Psychological contract is negatively related to professional knowledge,professional skills,professional competence and psychological characteristics.5、The correlation coefficient of psychological contract and turnover intention of ICU nurses was 0.435(p < 0.01).Psychological contract and turnover intention I,turnover intention II and turnover intention are all positive correlation.6、The correlation coefficient of ICU nurses’ competence and turnover intention was-0.124(p > 0.05),indicating that there was no correlation.7、The result of multiple regression shows that psychological contract is an important factor affecting turnover intention,and the variation of turnover intention 35.3% can be explained by "regular overtime" and "work preference".At the same time,psychological contract is also an important factor affecting the competency of ICU nurses.The basic condition of ICU nurses can explain the variation of ICU nurses’ competency 34.5%.The four dimensions of ICU nurse competency are not the main factors affecting the turnover of ICU nurses,but it is an important factor affecting the psychological contract of ICU nurses.The basic situation of nurses with ICU can explain the variation of psychological contract 40.5%.8、Structural equation model showed that the turnover intention in the path of competency,nurses,psychological contract,psychological contract nurses’ confidence interval average indirect path coefficient of 95% did not include 0,verify the psychological contract with the intermediary in the nurse competency and turnover intention for.Conclusion:1.The psychological contract of ICU nurses was at the middle level.The factors that affect the psychological contract of ICU nurses include age,title,ICU working life,the highest degree of education,regular overtime,work like or not,average monthly night shift and average monthly income.2.The scores of ICU nurses were higher,but the standard(total score > 174)was good.The factors affecting the competence of ICU nurses were age,title,duty,working life of ICU,marital status,children,ICU qualification certificate and average monthly income.3.The score of ICU nurses’ turnover intention is at the middle level,and the factors that affect turnover are only two aspects,such as regular overtime or not.4.ICU nurses’ psychological contract score was negatively correlated with their competency score,that is,the higher the degree of psychological contract perceived by nurses,the lower their competency.There is a positive correlation between the score of psychological contract of ICU nurses and the score of turnover intention,that is,the higher the nurses’ perceived psychological contract is,the stronger the nurses’ turnover intention is.However,there is no direct correlation between the competency of ICU nurses and their turnover intention.5.Nurses’ psychological contract has a mediating effect between competency and turnover intention and is a complete mediator. |
