Multiple myeloma is a malignant plasma cell disease,accounting for 1%of malignant tumors,accounting for13%of hematologic malignancies.In recent years,with the research of new drugs and treatment programs,treatment effect was significantly improved in MM patients and MM patients' life was prolonged.Clinical study found that more than 90%of MM patients occurred with MM-related bone disease(MBD)in disease process,including bone pain,pathological fractures,spinal compression,hypercalcemia,spinal compression and other adverse events.Studies have shown that MM patients with MBD,whose quality life has dropped significantly and the risk of death has increased significantly,has become the key factor in MM treatment effect.Studies have shown that MBD major pathogenesis of osteoblasts and osteoclasts activity imbalance that osteoclast activity increased and osteoblast activity was inhibited,which lead to bone formation loss.In addition,osteoclast activity increased plays a key role in the pathogenesis of MBD.Currently,Exploring the pathogenesis of MBD and its influencing factors in order that we improve the overall MM treatment effect.IL-17 is an inflammatory factor produced by activated CD4 + T helper cells,related to inflammation,autoimmune diseases,tumor and other pathological processes through the activation of MAPK and NF-?B pathway,which has been confirmed to participate in the pathogenesis of MBD and the specific mechanism is still unclear.MMP-13 is a member of the matrix metalloproteinases(MMPs),which play a role mainly by degrading the extracellular matrix.It is confirmed that the expression of MMP-13 in bone metastases of various solid tumors is higher than various solid tumors.However,MMP-13 is less studied in MBD.In this study,we examined the expression of MMP-13,IL-17 and NF-?B-p65 in the bone marrow biopsy of 47 patients with newly diagnosed MM,and further explored the possible pathogenesis of MBD and its association with MM.Objective:To investigate the expression of interleukin-17(IL-17)and metalloproteinase-13(MMP-13)in bone marrow biopsy of multiple myeloma(MM).Methods:The expressions of IL-17 and MMP-13 in bone marrow biopsy of 47 patients with MM and 10 cases as normal control groups were determined with immunohistochemistry method.Results:1.The mean optical density of IL-17 and MMP-13 in MM group was significantly higher than that in control groups(P<0.01).2.The mean optical density of IL-17 and MMP-13 positive expression in osteolytic lesions in MM group was significantly higher than that in patients without osteolytic lesions.3.The expression of NF-KB-p65 positive cells in MM group was significantly higher than that in control group(P<0.05);4.There was positive correlation between the IL-17 and MMP-13 expression(r =0.514,P<0.01)in MM group.Conclusion:The expression of IL-17 and MMP-13 in MM patients was significantly higher than that in the control group,and IL-17and MMP-13 were closely related to MBD,which may be involved in the pathogenesis of MBD through NF-?B pathway.
