Clinical Analysis Of Complex Regional Pain Syndrome

ObjectiveComplex regional pain syndrome(CRPS)has no specific clinical manifestations and sensitive diagnostic methods,leading to high rate of misdiagnosis.The clinical data of patients diagnosed with complex regional pain syndrome were collected to investigate clinical characteristics and treatments of this disease.MethodsClinical data of 51 patients with complex regional pain syndrome from April 2000 to September 2016 were retrospective analyzed,there were grouped according to disease duration and gender,we summarize the predisposing factors,concomitant disease,clinical characteristics and treatment method.Results1.General data:There were 22 cases(43.1%)with soft tissue injury history,25 cases(49%)with sleep disorders;Pain was the chief complaint for all patients,burning pain is the common complaint from patients.2.Clinical manifestation:Patients with the short duration(?1 years)were prone to have limb dysfunction(x2=4.404,P =0.036);Lower limb joints were more susceptible involved(x2 = 10.667,P= 0.001),and multiple joints involvement(x2 = 32.980,P =0.000)were more common,the number of women with osteoporosis was more than that of men(x2 =4.554,P=0.033),Male patients were prone to limb swelling(x2 =5.368,P=0.026).3.Imaging examination:CT was performed in 21 patients and 19(91%)of them showed typical patchy osteoporosis and bone cyst degeneration;Patients who underwent bone scan(ECT)all showed lesion radioactivity.4.Treatment plan:All patients received two or more than two kinds of medications,45(88.3%)of them received analgesic treatment,22 patients(43.1%)treated with bisphosphonates and 17(33.3%)with glucocorticoid,part of them(19 patients 37.2%)received sympathetic nerve block therapy.After comprehensive treatment that mentioned above,patients obtained satisfactory pain relief.ConclusionThe most common phenomenon of CRPS was mainly caught by Soft tissue injury,middle aged people were high risk population for CRPS.There were slightly higher numbers of women,but no significant difference on male and female ratio,Pain is the most common clinical symptom,the joints in lower limbs were involved mainly in CRPS.CT or ECT examination could guide clinical diagnosis.Multifaceted treatment may get satisfactory effect.
