| Objective:To analyse outcomes and prognosis factor of patients with Ewing sarcoma of bone treated by neo-adjuvant chemotherapy combined with local treatment.Methods:A retrospective analysis of 33 patients received at least 6 cycles chemotherapy with a regime consisting of vindesine,ifosfamide and doxorubicin from December 2005 to August 2016.There are 19 males and 14 females.The tumor located in pelvis in 11 cases,femoral in 10 cases,humerus in 4 cases,tibia in 3 cases,scapula in 2 cases and fibula,calcaneus,clavicle respectively in 1 case.Two in stage IIA,twenty-nine in stage IIB and two in stage III according to Enneking staging.The statistics about patients’ age,gender,tumor size,tumor stage and necrosis rate were recorded though medical records review,outpatient and telephone follow-up.Kaplan-Meier method was applicate to analyze various factors effect on the survival rate and log-rank test used for significance testing,P<0.05 is considered statistically significant.Results:The 5-year overall survival(OS)and 5-year event free survival(EFS)is 55.7%and 44.9%respectively.26 patients under widen excision,19 of them with tumor necrosis rate>90%which predicts good prognosis(P=0.006).Prognosis is associated with tumor stage,stage IIA predict good prognosis and stage III predict poor prognosis(P<0.001).Conclusion:The comprehensive treatment based on chemotherapy with a regime consisting of vindesine,ifosfamide and doxorubicin is an effective approach.Neo-adjuvant chemotherapy combined with surgery followed by chemotherapy is a recommendation for standard treatment.Early stage of tumor and good necrosis rate predict good prognosis. |
