Objective: To research the efficacy and safety of new cryotherapy device combined with tranexamic acid intra-articular injection on postoperative rehabilitation after total knee arthroplasty.Methods: A total of 90 patients with end-stage knee osteoarthritis,who were going to undergo total knee arthroplasty,were enrolled in the Guangxi Autonomous Region People's Hospital.The patients were divided into three groups named Group A,Group B,Group C according to designing by random control test.Each group included 30 patients respectively were carried different measures after receiving 1.0 g intravenous tranexamic acid.The patients in Group A had 10 ml normal saline injected into the articular cavity before incision suture,then closed the drainage tube for 4 hours.The patients in group had 10 ml normal saline with 1.0g tranexamic acid injected into the articular cavity before incision suture,then closed the drainage tube for 4 hours.The patients in group C use a new type of cryotherapy device when they go back the ward immediately apart from the same treatment as Group B.The drainage tubes were opened postoperative 4 hours in each group.We would note the following demographic details of the all patients:intraoperative bleeding volume,total postoperative blood loss,the total drainage volume,hidden blood loss,the decreased level Hb on the postoperative 3rd day,transfusion requirement,partial activated prothrombin time,prothrombin time on the postoperative 1st and 3rd day,pain score on the postoperative 24 th,48th hours,thighs swelling degree on postoperative 3rd and 7th day,the active joint range of motion on the postoperative 3rd day,at discharge,3rd month,knee joint function score,total days in hospital,numbers of patient with complications who had deep vein thrombosis of lower extremity,skin frostbite.Results: To compare some date as total postoperative blood loss,total drainage volume,hidden blood transfusion volume,the decreased level Hb on the 3rd day,blood requirement,pain score on the postoperative24 th,48th hours,thighs swelling degree on postoperative 3rd and 7th day,the active joint range of motion on the postoperative 3rd day,at discharge,3rd month,knee joint function score,total days in hospital.The results are significant.(P<0.05).However,There was no significant difference in intraoperative bleeding volumetotal blood loss,partial activated prothrombin time,prothrombin time on the 1st and 3rd day after operation.(P>0.05),and none of number patient with complications such as deep vein thrombosis and skin frostbite were found in 3 months follow-up.Conclusion:New cold therapy combined with tranexamic acid intra-articular injection had advantages in terms of blood loss,the measure could reduce swelling and pain,and increase joint motion,improve knee function score,promote early functional rehabilitation,decrease the time in hospital,improve the quality of life of patients.
