| Objective The aim of this study was to observe the clinical effect and prognosis of odontogenic cyst treated by fenestration decompression and complete curettage,and how to choose the cyst plug after the fenestration decompression.Methods Fifty-five patients with odontogenic cyst admitted to our hospital from October 2015 to December 2017 were divided into fenestrated decompression group and complete curettage group,of which 30 patients in fenestration group and 25 in curettage group.The operation time,intraoperative blood loss,postoperative pain and postoperative infection were compared between the patients of two groups,and the neurological complication and recurrence rate after operation in the fenestration group and the curettage group were compared.All the statistical analysis were perfomed by SPSS13.0 software.The main statistical methods included t test,Chi-square test.P <0.05 was considered statistically significant.We used different types of cyst plugs after fenestration decompression,and we compared the retention force,comfort and aesthetic aspects through the questionnaire.It was used for reference for how to choose the suitable cyst plug after fenestration decompression.Results Thirty patients in the fenestration group had an average operation time of 32.8 minutes,which was significantly shorter than that of the curettage group(63.96 minutes).The result was statistically significant(P <0.05).The average amount of bleeding in the fenestration group was 10.37 ml,which was significantly lower than that in the curettage group(103.08ml).The result was statistically significant(P <0.05).Two patients in the fenestration group had severe pain,which was significantly decreased compared with 15 patients in the scraping group.The results were statistically significant(P <0.05).In the fenestration group,there were 0 cases of postoperative infection in 30 patients,which were significantly lower than those in the curettage group after operation.The results were statistically significant(P <0.05).There were 0 cases occurred neuropathic complications in 25 patients after stage 2 operation of the fenestration group,which was lower than those in the curettage group.The results were statistically significant(P <0.05).There were 0 cases recurrence in 25 cases of fenestration after the second phase operation,which was lower than those in the curettage group.The result was statistically significant(P <0.05).As for comfort level,satisfaction rate of patients wearing denture type plug and orthodontic bracket type plug was 100%,satisfaction rate of patients wearing occlusal pad type plug was 50%,the satisfaction rate of patients wearing baseplate type plug was 71.4%.Evaluation of patients wearing orthodontic bracket type plug: 100% of patients considered a good retention;Patients wearing denture type plug evaluated: 71.4% of patients rated a good retention;Patients wearing baseplate type plug evaluated: 57.1% of patients considered that the retention is good;patients wearing occlusal pad type evaluated: 33.3% of the patients considered the retention is good.Conclusions 1.Fenestration decompression treated the odontogenic cysts could significantly reduce the operation time and reduce intraoperative bleeding,and relieve postoperative pain response,reduce postoperative infection rate.Neurological complications and postoperative recurrence rate were significantly lower than those in the complete curettage group.The ideal treatment for large odontogenic cysts is fenestration decompression.2.It is useful for the treatment of odontogenic cysts to use the cyst plug after fenestration decompression.Selecting the cystic plug needs to consider both retention and comfort and the patient’s own characteristics and many other factors.Retention force: orthodontic bracket type plug is the best,denture type plug and baseplate type plug are the next,occlusal pad type plug was the worst.The orthodontic bracket type plug and the denture type plug have the best comfort level,The baseplate type plug is the next,the occlusal pad type plug is the worst. |
