Objective:1.Analyze the distribution features of the structure and elements in the ancient Chinese medicine case and the modern Chinese medicine case.2.Summarize the structural features of Chinese medical records and form a directory of medical case elements with Chinese medicine characteristics and norms.Method:1.A Literature Review of the Structure and Elements of Chinese Medicine Cases.In order to sum up the historical changes of medical records in different historical periods,the paper collates the medical records in various periods and combines the history of Chinese medicine with the historical origins of the concept of medical records in different historical periods.Screening literature on the research of medical case structure and feature features in each historical period,sorting out the features of the medical case format,and establishing a directory of medical case structural elements.2.A Study on the Discovery of Medical History Structure and Features.Using bibliometric methods to screen 10 books of ancient medical records that are the most studied in contemporary medical records.Using expert consultation methods,select 2 expert medical record books that can represent the development level of contemporary Chinese medicine.A random sampling method was used to select 1,000 medical records in selected medical monographs and establish a database containing ancient and modern medical records.According to the directory of medical records established in the literature research,the structure and elements of medical records are extracted.Using the frequency statistics method,complex network method,Gephi modularity method,and Jaccard index method,the relationship between medical case structure,medical case factors,subject attributes,diagnosis times,and time is summarized.The core structure,core elements,and ancient and modern medical cases of the medical case are summarized.The difference between the different medical records of the different medical examinations eventually led to the formation of a medical record structural element list.Result:1.Literature research results①The origin of the medical case concept.The concept of the medical case has undergone the "zhenji" of the Han Dynasty,the "Pulse case",the "Classified Case" of the Ming Dynasty,the "disease-type" of the Qing Dynasty,and the contemporary"medical records".②Medical record format source stream.The development of the medical record format went from Biography of the Bian Que Cang Gong "zhenji" to Hansong "Six Methods of Medical Cases" to Wu Kun "Qi Shu Yi Yin" to Zhang Jingyue’s "Ten questions song" to Yu Chang’s "disease discussion".The evolution of the "new medical program" to He Lianchen eventually led to the formation of six medical format specifications.2.Medical Case Structure Analysis Results2.1 The core structure of the medical caseUsing frequency statistics and complex network methods to study the structure of medical records,15 core cases of medical records can be extracted,including 9 main core structures,Including Treatment information、Basic information、Current symptoms、Treatment analysis、Disease analysis、Name of disease syndrome、Pulse conditions、Chief complaint、therapeutic effect.And sub-core structure 6 items,respectively the incidence,tongue diagnosis,lesion process,diagnosis and treatment,observation,and past medical history.2.2 A Comparison of the Core Structures of Ancient and Modern Medical CasesThe core structure of the ancient medical case is similar to the core structure of the modern medical case,which is 80%.They are the same in eight of the 10 structures.The core structure of ancient medical records includes efficacy evaluation.The core structure of modem medical records includes tongue images.According to the strength of the complex network analysis results,the ancient medical case and the modern medical case were ranked separately.It can be found that the ancient medical case structure has the characteristics of attaching importance to the collection of basic patient information and disease name,and combining the expression of treatment information with disease analysis.However,modern medical records pay more attention to the collection of clinical symptoms such as symptoms,pulse,and tongue.2.3 The Distribution Characteristics of Medical Case StructureThe order of the medical records in different departments is basically the same,which infers that the structure of the medical case has nothing to do with the department.2.4 Comparison of medical structure distribution between first visit and referralCalculate the similarity between the two is 50%.The medical records for the first medical consultation and the medical medical case,the core elements of the medical case are 15 items.They are diagnosis of traditional Chinese medicine,pulse content,prescription composition,name,pathogenesis analysis，doctor information,main complaint content,prescription name,treatment method,treatment analysis,tongue coating,tongue body,treatment time,age,TCM syndromes diagnosis.These 15 elements can constitute a complete medical case.It is speculated that the medical case emphasizes its framework.Its core lies in highlighting the condition and treatment.3.2Analysis of medical record element categories based on community classification.The medical division of the factors is divided into three groups:group 4,group 12,and group 15.From the analysis of factor characteristics,the core elements of group 4 are mainly related to current symptoms,pulse diagnosis,tongue diagnosis,and treatment-related information.The elements in the community 12 are similar to those highlighted in modern medical records.The elements included in group 15 are element clusters based on medical analysis.3.3 Comparison of the Differences in the Core Elements of Ancient and Modern Medical Cases.The theme of the ancient medical case was the analysis of the disease.In addition,the ancient medical case focused on the construction of the medical case framework.The modern medical case pays attention to the collection of tongue and vein information,and requires basic information to be comprehensive.The importance of modern medical records for the analysis of disease conditions is less than that of ancient medical records.3.4The law of distribution of medical records in the first diagnosis and subsequent diagnosis.The first medical case pays attention to the construction of the medical case framework.The first consultation requires basic information(name,age,gender,doctor’s information),condition(conditions for diagnosis of TCM,diagnosis of TCM syndromes,main complaint content,and pulse content),and treatment information.(Formulation of prescription,name of prescription,treatment method),medical record analysis(analysis of disease and pathogenesis,treatment analysis),which attaches great importance to the introduction and analysis of the nature of the disease.The referral medical case pays attention to the curative effect,clinical manifestation and treatment information.Conclusion:This research combines the methods of literature research and data mining of medical records,aiming at the problem that the current utilization of medical records is not high,and the format of medical case structural elements is not standardized.Data mining on the structure and elements of ancient and modern medical cases from the perspective of disease type,times,and times,resulting in 9 medical case core structures and 15 core elements.And Clear a few issues.1.This study formed a 17-item structure and 75 elements of Chinese medical records.The 9 core structures of the medical case:Treatment information、Basic information、Current symptoms、Treatment analysis、Disease analysis、Name of disease syndrome、Pulse conditions、Chief complaint、therapeutic effect.The 15 core elements:diagnosis of traditional Chinese medicine,pulse content,prescription composition,name,pathogenesis analysis,doctor information,main complaint content,prescription name,treatment method,treatment analysis,tongue coating,tongue body,treatment time,age,TCM syndromes diagnosis.2.The Chinese medical case attaches great importance to the structural framework and its framework is composed of basic information of patients + clinical manifestations + treatment + effects + analysis.Disease conditions and treatment are the core of this framework,and other contents are its services.It shows that the medical case is a highly practical experience carrier.3.Comparing the structure of modern medical records with that of traditional Chinese medicine,we can find that although it is comprehensive,the emphasis is not enough,it is biased towards clinical records,and there is a lack of medical record analysis.The characteristics of Chinese medicine are not strong enough.4.The ancient medical case and the modern medical case are quite different in terms of structure and elements.The concrete manifestation is that the ancient medical case pays more attention to the analysis of disease and treatment than the modern medical case.The modern medical case focuses on the integrity of the diagnosis and treatment information.It shows that the connotation of modern medical records has changed.
