A Clinical Research Of The Effect On "Tong Guan Li Qiao Point Combination" For Dysphagia After Stroke

Background:Dysphagia after stroke is a common clinical complication,embodied in water choking and swallowing difficulties,which often cause pulmonary infection and respiratory infection.If these symptoms can’t timely be controlled,exacerbation will happen which include poor nutritional status,electrolyte metabolic abnormalities and disorder.When crisis occur such as dehydration and shock,the quality of survival and prognosis will be seriously affected.Modern medicine has no specific treatment for the disease,therapeutic principle mainly include repairing cranial nerve,promotion of circulation of the overall brain,controlling of the brain edema and so on.The treatment means include rehabilitation of the swallowing function training,nasal feeding by stomach tube,ice stimulation method and so on by using instrument.Although the above methods are affirm curative effect,but the treatments period length is long,patient compliance is not positive,and the recurrence rate will rise after quitting treatment.However,TCM therapy has unique advantages,among which acupuncture therapy has good clinical efficacy.Being the one of the best choices for TCM treatment,“Tong Guan Li Qiao needle punching”has obvious clinical efficacy.The study is further discussed and perfected on the basis of above discussion,so the “ Tong Guan Li Qiao Point Combination”have been put forward.Objective:The study has used scientific method for experimental design and data analysis.By comparing the clinical efficacy of“Tong Guan Li Qiao Point Combination”and conventional acupuncture therapy on dysphagia after stroke,the purpose of the study is to evaluate the efficacy of“Tong Guan Li Qiao Point Combination”on dysphagia after stroke and to explore the mechanisms of it and to provide evidence-based basis and do basic exploration for further study.Method:58 qualified patients according with the criteria were randomly and chronologically divided into the treat group(“Tong Guan Li Qiao Point Combination ”)and the control group(conventional acupuncture therapy).The even number were assigned to the treat group while the odd number were assigned to the control group.Finally,the treat group had 30 cases,the control group had 28 cases.All the cases were treated by therapeutic principle which include repairing cranial nerve,promotion of circulation of the overall brain,controlling of the brain edema and so on according to the guidelines for prevention and treatment of cerebrovascular diseases in China when necessary.Some qualified patients can acquire swallow training.In the treat group,the acupoints were mainly from Yin meridians and governor meridian.The treat group was focus on needling angle and depth,manipulation of acupuncture and clinical manifestation,while emphasized the most dose-effect relationship.The control group adopted conventional acupuncture therapy.After 42 days(3 courses),the six international standard scoring scales were used to compare the treat group(“Tong Guan Li Qiao Point Combination”)and the control group(conventional acupuncture therapy)on dysphagia after stroke.The expulsion cases and adverse events had to be recorded.Result:There was no statistically significant difference between the two groups of gender,age,course of disease,weight,height,and the scores of the six standard scoring scales before treatment,which could be further analyzed.After treatment,the scores of the six standard scoring scales of the two groups were both higher than before the treatment,with statistical difference and statistically significant difference.The difference between the two groups was more obvious than the control group,and the difference was statistically significant.The efficacy of the two groups was better than that of the control group,and the difference was statistically significance.Compared the score difference of the six standard scoring scales between the treat group(“Tong Guan Li Qiao Point Combination ”)and the control group(conventional acupuncture therapy),the treat group was better than the control group with statistically significance.Compared the curative effect after therapy between the treat group(“Tong Guan Li Qiao Point Combination”)and the control group(conventional acupuncture therapy)with six standard scoring scales,the treat group was better than the control group with statistically significance.The safety assessment after therapy of both groups was normal.Conclusion:In this research,the effect of the treat group(“Tong Guan Li Qiao Point Combination”)for dysphagia after stroke was much better than that of control group(conventional acupuncture therapy).“Tong Guan Li Qiao Point Combination” can improve the swallowing function and reduce the complications significantly,which can improve the survival rate.“Tong Guan Li Qiao Point Combination ” has the advantage of safety assessment,reliable patient compliance,stable operability and so on.The research have no complications and adverse reactions,which can be used in clinical treatment for dysphagia after stroke diffusely.
